临床药师参与1例脑出血合并静脉血栓栓塞症患者的药学监护

Pharmaceutical Care for One Patient with Tracerebral Hemorrhage Complicated with Venous Thromboembolism

探讨临床药师在脑出血合并静脉血栓栓塞症(VTE)患者的药物治疗中的作用。临床药师对降压药物、抗凝药物使用及药物不良反应等方面全程实施药学监护,向医师提出合理建议,在参与患者诊疗过程中,从药学角度协助医师优化用药方案,能一定程度提高临床疗效,改善了临床用药的合理性。

The role of clinical pharmacists in the treatment of patients with tracerebral hemorrhage complicatd with venous thromboembolism(VTE)was discussed.The whole course of drug monitoring for antihypertensives,anticoagulant agents and adverse drug reactions were analyzed.Clinical pharmacists put forward reasonable suggestions for drug use,participated in the diagnosis and treatment of the patient and improved the clinical efficacy.The curative effect was also improved to a certain extent,as well as the rationality of clinical medication.
