维生素D受体基因多态性与青少年结核易感性的研究

Correlation between vitamin D receptor genetic polymorphism and susceptibility to tuberculosis in adolescents

目的探讨维生素D受体(VDR)基因多态性与青少年肺结核(PTB)易感性的关系,为青少年肺结核病的防治手段提供遗传学理论依据。方法在合肥市及山南市共采集青少年肺结核病患者96例(病例组)、健康对照123例(对照组)的外周静脉血。应用限制性片段长度多态性聚合酶链反应(PCR-RFLP)分析方法检测VDR基因位点(ApaI、BsmI、FokI、TaqI)的基因型,并用χ~2检验分别分析两地区青少年VDR基因多态性与结核易感间的关系。结果两地区病例组BsmI位点基因型与对照组相应基因型比较,差异有统计学意义(合肥市:χ~2=19.948,P<0.001,山南市:χ~2=38.485,P<0.001);两地区病例组FokI位点基因型与对照组相应基因型比较,差异有统计学意义(合肥市:χ~2=7.231,P=0.027,山南市:χ~2=21.781,P<0.001)。而两地区病例组ApaI及TaqI位点基因型与对照组相应基因型比较,差异无统计学意义(P>0.05)。结论VDR基因BsmI及FokI位点多态性与青少年肺结核易感有关,BsmI位点Bb、bb基因型及FokI位点Ff、ff基因型增加青少年肺结核病易感风险。

Objective To investigate the relationship between vitamin D receptor(VDR) gene polymorphism and susceptibility to pulmonary tuberculosis( PTB) in adolescents,and to provide genetic theoretical basis for prevention and treatment of PTB in adolescents. Methods Peripheral venous blood was collected from 96 patients with pulmonary tuberculosis( as the case group) and 123 healthy controls( as the control group) in Hefei and Shannan.The genotypes of VDR( ApaI,BsmI,FokI,TaqI) were detected by PCR-RFLP,and Chi-square test was used to analyze the relationship between VDR gene polymorphism and TB susceptibility in adolescents from two regions. Results The polymorphism of BsmI locus was significantly different between the case group and the control group in Hefei and Shannan( Hefei: χ~2= 19. 948,P < 0. 001,Shannan: χ~2= 38. 485,P < 0. 001). The polymorphism of FokI locus was significantly different between the case group and the control group in Hefei and Shannan( Hefei: χ~2=7. 231,P = 0. 027,Shannan: χ~2= 21. 781,P < 0. 001). But the polymorphisms of ApaI and TaqI locus showed no significant difference between the case group and the control group( P > 0. 05). Conclusion VDR gene BsmI and FokI locus polymorphism are related to susceptibility to tuberculosis in adolescents,the Bb,bb,Ff and ff genotypes increase the susceptibility to tuberculosis in adolescents.
