血清癌胚抗原与糖类抗原199及糖类抗原724水平检测诊断结直肠癌的临床分析

Clinical Analysis of Serum CEA,CA199 and CA724 Levels in Diagnosis of Colorectal Cancer

目的探讨血清癌胚抗原(CEA)、糖类抗原199(CAl99)、糖类抗原724(CA724)水平检测诊断结直肠癌的临床价值。方法回顾性分析2019年9月—2020年3月天津医科大学肿瘤医院收治的100例结直肠病变患者的临床资料,根据病变程度的不同分为A组(结直肠癌,30例)和B组(结直肠良性病变,70例)。两组患者均进行血清CEA、CA199、CA724水平检测,分析其在诊断结直肠癌中的价值。结果A组血清CEA、CA199、CA724水平均明显高于B组,差异有统计学意义(P<0.05);绘制ROC曲线,血清CEA、CA199、CA724以及联合检测诊断不同病变程度的结直肠病灶AUC分别为0.763、0.879、0.694、0.908,其中联合检测AUC>0.9,诊断价值最好。结论结直肠癌患者血清CEA、CA199、CA724水平明显高于结直肠良性病变患者,且血清CEA、CA199、CA724联合检测较单一指标检测诊断结直肠癌的效果更好,预测价值更高。

Objective To investigate the clinical value of serum carcinoembryonic antigen(CEA),carbohydrate antigen 199(CAl99),carbohydrate antigen 724(CA724)in the diagnosis of colorectal cancer.Methods The clinical data of 100 patients with colorectal disease admitted to the hospital from September,2019 to March,2020 were retrospectively analyzed,and were divided into group A(colorectal cancer,30 cases)and group B(colorectal cancer benign lesions,70 cases)according to the degree of disease.Serum CEA,CA199 and CA724 levels were detected in both groups,and their value in diagnosing colorectal cancer was analyzed.Results The levels of serum CEA,CA199 and CA724 in group A were significantly higher than those in group B,and the difference was statistically significant(P<0.05).ROC curve was drawn.Serum CEA,CA199,CA724 and combined detection of colorectal lesions AUC of different lesions was 0.763,0.879,0.694 and 0.908,respectively.The combined detection AUC was>0.9,and it had the best diagnostic value.Conclusion Serum levels of CEA,CA199,and CA724 in patients with colorectal cancer are significantly higher than those in patients with benign colonic lesions,and the combined detection of serum CEA,CA199,and CA724 is better than a single indicator in the diagnosis of colorectal cancer,and the prediction value is higher.
