股动脉穿刺后Angio-seal血管闭合装置与手工压迫的短期并发症比较

Comparison of short-term complications between Angio-seal vascular closure device and manual compression after femoral artery puncture

目的:比较经股动脉穿刺经皮冠状动脉介入治疗(PCI)术后Angio-seal血管闭合装置(VCD)和手工压迫(MC)的患者舒适性和短期局部并发症情况。方法:根据术后闭合股动脉穿刺伤口方式,于我院行PCI术的120例患者被分为MC组(48例,术后行MC)与Angio-seal组(72例,PCI术后使用Angio-seal VCD)。观察比较两组术后6h内疼痛程度数值评定量表(PI-NRS)评分及术后48h内伤口局部并发症发生率。结果:与MC组比较,Angio-seal组术后6h内腰背部PI-NRS评分[2(1,4)分比1(0,1)分]显著降低,术后48h内皮下瘀斑发生率(10.4%比25.0%)显著升高(P依次分别为<0.01,<0.05)。结论:Angio-seal VCD可显著提高患者术后舒适性,但其术后皮下瘀斑发生率显著升高。

To compare comfort and incidence of short-term local complications between Angio-seal vascular closure device(VCD)and manual compression(MC)in patients undergoing percutaneous coronary intervention(PCI)via femoral artery puncture.Methods:According to method of closing femoral artery puncture wound after PCI,a total of 120 patients undergoing PCI in our hospital were divided into MC group(n=48,received MC after PCI)and Angio-seal group(n=72,received Angio-seal VCD after PCI).Pain intensity numerical rating scale(PI-NRS)score within 6h after PCI and incidence rate of local wound complications within 48h after PCI were observed and compared between two groups.Results:Compared with MC group,there was significant reduction in PI-NRS scores of waist and back within 6h after PCI[2(1,4)scores vs.1(0,1)scores],and significant rise in incidence rate of subcutaneous ecchymosis within 48h after PCI(10.4%vs.25.0%)in Angio-seal group(P respectively<0.01,<0.05 in turn).Conclusion:Angio-seal VCD can significantly improve comfort in patients after PCI,however,the incidence rate of subcutaneous ecchymosis significantly rises after PCI.
