三级医院-社区-家庭心脏康复体系对稳定期慢性心力衰竭患者心功能和活动耐量的改善效果

Improvement effect of tertiary hospital-community-family cardiac rehabilitation system on cardiac function and exercise tolerance in patients with stable chronic heart failure

目的探讨三级医院-社区-家庭心脏康复体系对稳定期慢性心力衰竭患者心功能和活动耐量的改善效果。方法将150例慢性稳定期心力衰竭患者随机分为观察组和对照组,每组75例。对照组出院后采用传统方式进行康复,观察组出院后在传统康复方式基础上采用三级医院-社区-家庭心脏康复体系一体化管理模式进行康复。比较两组患者康复治疗1年后的心功能疗效,以及康复治疗前和康复治疗1年后两组患者6分钟步行试验(6MWT)距离及代谢当量。结果康复治疗1年后,观察组心功能总有效率高于对照组,两组患者6MWT距离、代谢当量均高于治疗前,且观察组均高于对照组(均P<0.05)。结论对于稳定期慢性心力衰竭患者,采用三级医院-社区-家庭心脏康复管理体系可明显改善心功能,增加6MWT距离,提高活动耐量。

Objective To investigate the improvement effect of tertiary hospital-community-family cardiac rehabilitation system on cardiac function and exercise tolerance in patients with stable chronic heart failure.Methods A total of 150 patients with stable chronic heart failure were randomly divided into observation group(n=75)and control group(n=75).The control group received rehabilitation via traditional approach after discharge from hospital,and the observation group received rehabilitation using the integrated management mode of tertiary hospital-community-family cardiac rehabilitation system on the basis of rehabilitation via traditional approach after discharge from hospital.Efficacy of 1-year rehabilitation treatment on cardiac function as well as 6-minute walking test(6MWT)distances and metabolic equivalents before rehabilitation treatment and one year after rehabilitation treatment was compared between the two groups.Results After one year of rehabilitation treatment,the total efficacy on cardiac function in the observation group was higher than that in the control group,6MWT distance and metabolic equivalent were higher in both groups than those before treatment,which were higher in the observation group than in the control group(all P<0.05).Conclusion For patients with stable chronic heart failure,the application of tertiary hospital-community-family cardiac rehabilitation management system can significantly improve cardiac function,increase 6MWT distance and improve exercise tolerance.
