腹腔镜手术与开腹手术治疗结直肠癌患者的效果比较

Comparison of effects of laparoscopic surgery and open surgery in treatment of patients with colorectal cancer

目的:比较腹腔镜手术与开腹手术治疗结直肠癌患者的效果。方法:选取2014年1月至2015年1月接受开腹手术治疗的75例结直肠癌患者,设为对照组;选取2014年3月至2015年3月接受腹腔镜手术治疗的85例结直肠癌患者,设为观察组。比较两组手术相关临床指标水平、术后24 h疼痛程度和并发症发生率,随访5年复发率及无病生存率。结果:观察组术中出血量少于对照组,手术时间、术后首次排气时间和住院时间均短于对照组,差异有统计学意义(P<0.05);观察组术后视觉模拟评分法评分为(3.25±1.65)分,明显低于对照组的(5.32±2.04)分,差异有统计学意义(P<0.05);观察组并发症发生率为8.24%(7/85),明显低于对照组的29.33%(22/75),差异有统计学意义(P<0.05);随访5年,两组复发率和无病生存率比较,差异均无统计学意义(P>0.05)。结论:腹腔镜手术治疗结直肠癌患者,可减少术中出血量,缩短手术时间、术后首次排气时间和住院时间,降低疼痛评分和并发症发生率,其效果优于开腹手术治疗效果。

Objective:To compare effects of laparoscopic surgery and open surgery in treatment of patients with colorectal cancer.Methods:75 patients with colorectal cancer,who underwent open surgery from January 2014 to January 2015,were set as control group,while 85 patients with colorectal cancer,who underwent laparoscopic surgery from March 2014 to March 2015,were used as observation group.The level of surgery-related clinical indicators,the pain degree and complication rate 24 h after the surgery,the recurrence rate and disease-free survival rate at 5-year follow-up were compared between the two groups.Results:The intraoperative blood loss of the observation group was less than that of the control group;the operation time,first postoperative exhaust time and hospital stay were shorter than those of the control group;and the differences were statistically significant(P<0.05).The postoperative visual analogue scoring score of the observation group was(3.25±1.65)points,which was significantly lower than(5.32±2.04)points of the control group,and the difference was statistically significant(P<0.05).The complication rate in the observation group was 8.24%(7/85),which was significantly lower than 29.33%(22/75)in the control group,and the difference was statistically significant(P<0.05).At 5-year follow-up,there were no significant differences in the recurrence rate and disease-free survival rate between the two groups(P>0.05).Conclusions:Laparoscopic surgery for the patients with colorectal cancer can reduce the intraoperative blood loss,shorten the operation time,the first postoperative exhaust time and the hospital stay,and reduce the pain score and the incidence of complications.Moreover,it is superior to open surgery.
