摘要
目的探讨糖尿病前期患者血清炎症因子、血管内皮损伤因子水平与绝经后骨质疏松症(PMOP)相关性。方法本研究收集2019年3月至2020年3月成都市第一人民医院内分泌科门诊及住院部符合条件的93例患者为研究对象,并分为PMOP组与非PMOP组。采集患者基本特征、骨密度(BMD)、生化指标、空腹血清炎症因子及血管内皮损伤因子水平。BMD与血清炎症因子、血管内皮损伤因子的相关性采用Pearson相关分析,糖尿病前期合并PMOP危险因素的分析采用非条件Logistic回归。结果糖尿病前期合并PMOP患者血清血清肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)、血管性血友病因子(vWF)和内皮素-1(ET-1)水平高于非PMOP组(P<0.05),Pearson相关分析显示,BMD与血清炎症因子、血管内皮损伤因子水平呈负相关(r<0,P<0.05);非条件Logistic回归分析显示,最终变量体质指数(BMI)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、TNF-α)、hs-CRP、v WF和ET-1进入回归方程(P<0.05),OR的95%置信区间分别是(1.26,1.46)、(1.54,1.74)、(1.23,1.42)、(1.62,1.96)、(1.56,1.98)、(1.33,1.87)和(1.55,2.37)。同时,BMI水平减低,糖尿病前期合并PMOP的风险越大。结论糖尿病前期患者血清炎症因子、内皮损伤因子与骨质疏松密切相关,为今后探寻糖尿病前期患者合并PMOP的易感生物标志物提供依据。
Objective To explore the relationship between serum inflammatory factors,vascular endothelial injury factors and postmenopausal osteoporosis(PMOP)in patients with prediabetes.Methods This study collected 93 patients who from the outpatient and inpatient departments of the Endocrinology Department of Chengdu First People’s Hospital from March 2019 to March 2020,and divided them into PMOP and non-PMOP groups.The basic characteristics of patients,bone mineral density(BMD),biochemical indicators,fasting serum inflammatory factors and vascular endothelial injury factor levels were collected.The correlation between BMD and serum inflammatory factors and vascular endothelial injury factors was analyzed by Pearson correlation,and the risk factors of prediabetes combined with PMOP were analyzed by unconditional Logistic regression.Results High serum tumor necrosis factor-α(TNF-α),high-sensitivity C-reactive protein(hs-CRP),von Willebrand factor(vWF)and endothelin-1(ET-1)levels in patients with prediabetes and PMOP In the non-PMOP group(P<0.05),Pearson correlation analysis showed that BMD was negatively correlated with serum inflammatory factors and vascular endothelial injury factor levels(r<0,P<0.05);unconditional Logistic regression analysis showed that the final variable body mass index(BMI),total cholesterol(TC),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),TNF-α),hs-CRP,v WF and ET-1 enter the regression equation(P<0.05),95%confidence intervals of OR were(1.26,1.46),(1.54,1.74),(1.23,1.42),(1.62,1.96),(1.56,1.98),(1.33,1.87)and(1.55,2.37).The lower the BMI level,the greater the risk of PMOP in patients with prediabetes.Conclusion Serum inflammatory factors and endothelial injury factors in pre-diabetic patients were closely related to osteoporosis,which provided a basis for exploring the susceptibility biomarkers of pre-diabetic patients with PMOP in the future.
