医共体模式下高血压管理现况及分析

Current situation and analysis of hypertension management under medical community

目的医共体是深化医疗卫生改革、健全分级诊疗系统的有效组织形式,对于慢病管理具有治疗延续性、管理动态化、双向转诊快等优势,在高血压等慢病防治及管理中取得了一定成效。但在当前具体实践中尚存在一些问题。本研究综述了医共体模式下高血压病的管理现状、问题分析及对策,对未来医共体模式下高血压病管理提出了展望与建议。利用中国知网、万方数据库、PubMed、Google scholr中英文数据库,以"医共体""医联体""高血压""分级诊疗"为关键词,检索2012年1月至2019年12月相关文献。纳入标准:(1)医共体模式管理的现况及进展;(2)高血压管理。剔除标准:(1)研究方法存在疑问,结论真实性不可靠;(2)研究资料缺失或质量较差。最终35篇文献纳入分析。医共体模式对于高血压等慢病管理具有治疗延续性、管理动态化、双向转诊快等优势,在高血压防治过程中取得了一定成效。但仍存在诊疗服务及转诊协调能力不足、延续性不够、信息互通不畅等问题。"医共体"模式对于高血压患者健康管理具有较好成效,具有较大的推广应用价值。

OBJECTIVE Medical community is an effective organizational form to deepen the reform of medical and health care and improve the graded diagnosis and treatment system.It has the advantages of treatment continuity,dynamic management and quick two-way referral for chronic disease management,and has made some achievements in the prevention and management of chronic diseases such as hypertension.However,there are still some problems in the practice of the current medical community model.This paper summarizes the current situation,problem analysis and countermeasures of hypertension management under the mode of medical community,and puts forward the prospect and suggestions for the future management of hypertension under the mode of medical community.Papers were searched by using CNKI,WanFang database,PubMed,Google scholr Chinese and English databases,with the keywords of"medical community","medical association","hypertension"and"graded diagnosis and treatment".The relevant literatures from January 2012 to December 2019 were searched.Inclusion criteria:(1)current situation and progress of medical community model management;(2)hypertension management.Exclusion criteria:(1)there are doubts about the research methods and the authenticity of the conclusions is not reliable;(2)the research materials are missing or the quality is poor.Finally,35 articles were included in the analysis.The medical community model had the advantages of continuous treatment,dynamic management and quick two-way referral for chronic diseases such as hypertension,and had achieved some results in the prevention and treatment of hypertension.However,there were still some problems,such as the lack of coordination,continuity and information exchange.The"health management platform of medical community"had a good effect on the health management of hypertension patients,and had a great application value.
