摘要
目的探讨水飞蓟素对糖尿病肾病蛋白尿水平的影响。方法在PubMed、EMBASE、Cochrane、Web of Science、中国知网、万方数据库和维普中文期刊数据库中检索水飞蓟素治疗糖尿病肾病的文献。根据Cochrane手册对纳入文献进行系统评价,使用RevMan 5.3软件对水飞蓟素是否影响糖尿病肾病蛋白尿水平进行Meta分析。结果 共纳入6篇随机对照试验文献,纳入糖尿病肾病病人426例,其中水飞蓟组213例,对照组213例。与对照组相比,水飞蓟组病人的蛋白尿水平显著降低(SMD=-2.04,95%CI=-3.23^-0.86,P<0.05)。在微量蛋白尿期,水飞蓟素对蛋白尿水平有较好的改善效果(SMD=-3.51,95%CI=-5.49^-1.54,P<0.05);而在大量蛋白尿期,则未观察到水飞蓟素对蛋白尿的改善作用。结论 在微量蛋白尿期,水飞蓟素能显著改善糖尿病肾病病人的蛋白尿水平。
ObjectiveTo investigate the effect of silymarin on proteinuria level in diabetic kidney disease(DKD).MethodsPubMed,EMBASE,Cochrane,Web of Science,CNKI,Wanfang Data,and VIP were searched for the articles on silymarin in the treatment of DKD.A systematic review was conducted for the articles according to Cochrane handbook,and RevMan 5.3 software was used to perform a Meta-analysis of the effect of silymarin on proteinuria level in DKD.ResultsA total of 6 randomized controlled trials were included,with 426 patients in total(213 in the silymarin group and 213 in the control group).Compared with the control group,the silymarin group had a significant reduction in the level of proteinuria(SMD=-2.04,95%CI=-3.23 to-0.86,P<0.05).In the period of microproteinuria,silymarin had a better effect on improving proteinuria(SMD=-3.51,95%CI=-5.49 to-1.54,P<0.05),while in the period of macroproteinuria,the effect of silymarin in improving proteinuria was not observed.ConclusionSilymarin can significantly improve proteinuria level in patient with DKD during the pe-riod of microalbuminuria.
