摘要
免疫治疗是目前肿瘤领域研究热点。免疫治疗在获得显著疗效的同时可能引起一系列免疫相关不良反应,可累及病人全身各个系统。我院收治1例宫颈癌病人行帕博利珠单抗治疗后出现免疫相关性结肠炎,表现为腹痛、腹泻、黏液血便,抗生素治疗无效,给予大剂量糖皮质激素治疗后好转。本文对该病例临床资料进行分析,并复习相关文献,探讨免疫相关性结肠炎的发病机制、临床特点、鉴别诊断及治疗原则。
Immunotherapy is currently a hot topic in the field of tumor research.Immunotherapy can achieve a marked clinical effect and may also cause a series of immune-related adverse reactions,which may involve all systems of the body.One patient with cervical cancer was admitted to our hospital and developed immune-mediated colitis after pembrolizumab treatment,with the manifestations of abdominal pain,diarrhea,and mucosanguineous feces.The patient had no response to antibiotics and was improved after being treated with a large dose of glucocorticoid.This article analyzes and reports the clinical data of this case and reviews related articles to discuss the pathogenesis,clinical features,differential diagnosis,and treatment principles of immune-mediated colitis.
