多奈哌齐对阿尔茨海默病患者血清和脑脊液中高迁移率族蛋白B1表达的近期影响

Effects of Donepezil treatment on the expression of high-mobility group box 1 in serum and cerebrospinal fluid in Alzheimei^s disease patients

目的探讨多奈哌齐对阿尔茨海默病患者血清和脑脊液中高迁移率族蛋白B1(HMGB1)表达的影响。方法单中心观察性研究,选择我院2017年3月至2019年5月收治的阿尔茨海默病患者120例,采用随机数字表法随机分为对照组与多奈哌齐组(各60例)。对照组患者给予常规改善脑功能代谢药物治疗,多奈哌齐组在对照组用药基础上加用盐酸多奈哌齐治疗(5 mg/d)。比较两组治疗前、后1个月血清和脑脊液HMGB1表达水平、老年性痴呆评定量表(ADAS-Cog)、简明智能量表(MMSE)、日常生活能力量表(ADL)和神经精神科问卷(NPI)评分的变化。结果多奈哌齐组患者治疗后ADAS-Cog评分低于对照组[(25.2±2.7)分比(33.4±3.6)分(P<0.05)];MMSE评分高于对照组[(23.3±2.1)分比(19.4±1.9)分(P<0.05)]。多奈哌齐组患者治疗后ADL评分高于对照组,(56.3±2.1)分比(46.9±1.6)分(P<0.05),NPI评分低于对照组(16.2±2.3)分比(22.3±2.6)分(P<0.05)。多奈哌齐组治疗后血清和脑脊液HMGB1水平均低于对照组,(45.3±5.3)μg/L比(56.4±4.4)μg/L、(39.2±3.3)μg/L比(47.1±3.9)μg/L,差异有统计学意义(均P<0.05)。结论多奈哌齐可下调阿尔茨海默病患者血清和脑脊液中HMGB1水平,多奈哌齐对阿尔茨海默病患者认知功能的改善可能与HMGB1的表达调节有关。

Objective To invevstigate the effect of Donepezil treatment on the expression of high mobility group box 1 protein(HMGB1)in serum and cerebrospinal fluid in Alzheimer's disease patients.Methods This is a single-center observational stady.A total of 120 Alzheimer’s disease patients admitted in our hospital from March 2017 to may 2019 were randomly divided into the control group receiving the routine drug therapy(n=60)and the Donepezil group receiving Donepezil hydrochloride(5 mg/d)as an add-on to medicine of control group(n=60).The expression levels of HMGB1 in serum and cerebrospinal fluid,Alzheimer's disease assessment scale(ADAS-Cog),minimental state examination(MMSE)scores,activities of daily living(ADL) and neuropsychiatric inventory(NPI)were compared before versus after 1 month of treatment.Results After the Donepezil treatment,the ADAS-Cog score was lower,MMSE score was higher,ADL score was higher and NPI score was lower in the Donepezil group than in the control group(25.2+2.7 vs.33.4zb 3.6,23.3± 2.1 is.19.4±1.9,56.3±2.1 vs.46.9±1.6,16.2±2.3 vs.22.3±2.6,P<0.05).After the Donepezil treatment,the levels of HMGB1 in serum[(45.3±5.3)μg/L vs.(56.3±4.4)μg/L]and in cerebrospinal fluid[(39.2±3.3)μg/L vs.(47.1±3.9)μg/L]were lower in the Donepezil group than in the control group(all P<0.05).Conclusions Donepezil treatment can downregulate the HMGB1 expression levels in serum and cerebrospinal fluid in Alzheimers disease patients,which may related to the improvement of cognitive function in Alzheimers disease patients.
