麻黄附子细辛汤治疗老年妇女压力性尿失禁的临床效果分析

Analysis on the clinical effect of Mahuang Fuzi Xixin Decoction in treatment of stress urinary incontinence in elderly women

目的观察麻黄附子细辛汤治疗中老年妇女尿失禁的临床疗效。方法选取2017年1月-2018年6月该院收治的160例尿失禁患者为研究对象,按照随机数字表法分为对照组与观察组,各80例。对照组患者给予酒石酸托特罗定治疗,每次2 mg,每日2次,观察组在对照组治疗的基础上加用麻黄附子细辛汤治疗,每日1剂,水煎温服,每日2次,两组均连续治疗3个月。比较两组患者临床疗效及相关指标改善情况。结果对照组有效率为66.25%,观察组有效率为88.75%,两组有效率比较,差异有统计学意义(P<0.05);两组失禁频率、失禁程度、夜尿次数比较,治疗后两组患者的上述指标都出现大幅度好转,与治疗之前指标水平比较,差异有统计学意义(P<0.05),且就好转程度而言,观察组较之对照组表现更优(P<0.05)。治疗后两组患者的ICI-Q-SF评分都出现了大幅度下降(P<0.05),而I-QOL评分方面则出现了大幅度上升(P<0.05),且就症状的改善情况来看,观察组较之对照组表现更优,差异有统计学意义(P<0.05)。两组患者于治疗后的阴道压力,不论是静息压还是收缩压都了大幅度上升,相较于治疗之前,差异有统计学意义(P<0.05),且相较于对照组,观察组患者阴道压力显著升高,差异有统计学意义(P<0.05)。治疗后,SFL、BC和PMUC等动力学参数都出现了大幅度上升,相较于治疗之前,差异有统计学意义(P<0.05)。且与对照组相比,观察组尿动力学参数的上升表现更加显著,差异有统计学意义(P<0.05)。结论麻黄附子细辛汤可减少中老年妇女尿失禁患者尿失禁次数,明显增加其阴道压力,改善其尿动力学参数,提高其生活质量。

Objective To observe the clinical effect of Mahuang Fuzi Xixin Decoction on urinary incontinence in middle-aged and elderly women.Methods a total of 160 patients with urinary incontinence admitted to our hospital from January 2017 to June 2018 were selected as study subjects.According to the random number table method,they were divided into control group and observation group with80 patients each.Patients in the control group were given toltrodine tartrate 2 mg each time,twice a day.On the basis of the treatment in the control group,Mahuang Fuzi Xixin Decoction was added to the treatment in the observation group,1 dose per day,decocted in water and taken twice a day,and both groups were treated for 3 months continuously.The clinical efficacy and the improvement of related indexes were compared between the two groups.Results The effective rate was 66.25%in the control group and 88.75%in the observation group.The difference was statistically significant(P<0.05).Compared with the indicators of incontinence frequency,incontinence degree,and nocturia frequency,the two groups showed a significant improvement after treatment.Compared with the indicators before treatment,P<0.05.In terms of the improvement degree,the observation group showed a better performance than the control group(P<0.05).After treatment,the ICI-Q-SF score of both groups showed a significant decrease(P<0.05),while the I-QOL score showed a significant increase(P<0.05).In terms of the improvement of symptoms,the observation group showed a significantly better performance than the control group(P<0.05).After treatment,vaginal pressure in both the resting pressure and systolic pressure increased significantly in both groups,P<0.05 compared with that before treatment,and significantly increased in the observation group compared with that in the control group(P<0.05).After treatment,SFL,BC,PMUC and other dynamic parameters were significantly increased,compared with that before treatment,P<0.05.Compared with the control group,the increase of urodynamic parameters in the observation group was more significant(P<0.05).Conclusion Mahuang Fuzi Xixin Decoction can reduce the frequency of urinary incontinence in middle-aged and elderly women patients,significantly increase their vaginal pressure,improve their urodynamic parameters,improve their quality of life.
