摘要
目的研究微小RNA-7(microRNA-7,miRNA-7)在原发性干燥综合征(primary Sjogren′s syndrome,pSS)的B淋巴细胞中的作用及与疾病活动度的关系。方法选取2017年5月至2019年9月青海省人民医院门诊及住院的20名pSS患者作为病例组,选择同期20名健康体检者作为对照组,提取外周血单个核细胞,磁珠分选法分离出B淋巴细胞。逆转录-聚合酶链反应(reverse transcription polymerase chain reaction,RT-PCR)检测病例组及对照组人群B淋巴细胞及血浆中miRNA-7相对表达水平,运用相关性分析研究B淋巴细胞中miRNA-7的表达与疾病活动度间的关系。结果病例组的B淋巴细胞中miRNA-7较对照组明显上调[(0.53±0.17)比(0.39±0.11)],差异具有统计学意义(P<0.05)。血浆中miRNA-7表达在病例组(0.41±0.14)与对照组(0.33±0.13)相比差异无统计学意义(P>0.05)。病例组B淋巴细胞中miRNA-7较血浆中明显上调[(0.53±0.17)比(0.41±0.14)],差异具有统计学意义(P<0.05),但miRNA-7在B淋巴细胞中与血浆中的表达水平无明显相关性(P>0.05)。miRNA-7相对表达量与EULAR干燥综合征活动度评分(EULAR Sjogren’s synddrome disease activity index,ESSDAI)(r=0.635,P<0.05)、抗干燥综合征相关抗原B(Sjogren’s synddrome related antigen B,SSB)抗体(rs=0.490,P<0.05)、免疫球蛋白G(immunoglobulin G,IgG)(r=0.455,P<0.05)、免疫球蛋白A(immunoglobulin A,IgA)(r=0.451,P<0.05)正相关,与补体4(complement 4,C4)(r=-0.643,P<0.05)、白细胞计数(white blood cell count,WBC)(rs=-0.489,P<0.05)负相关,与SSB抗体、免疫球蛋白M(immunoglobulin M,IgM)、C-反应蛋白(C-reaction protein,CRP)、血沉(erythrocyte sedimentation rate,ERS)、血小板计数(platelet count,PLT)、血红蛋白(helicorubin,Hb)、类风湿因子(rheumatoid factor,RF)、抗核抗体(anticardiolipin antibodies,ANA)无明显相关性(P>0.05)。结论miRNA-7可能参与原发性干燥综合征的发生和发展过程,未来可能成为治疗原发性干燥综合征的靶点之一,同时可能成为临床监控疾病活动度指标。
Objective To study the role of microRNA-7(miRNA-7)in B lymphocytes of primary Sjögren's syndrome(pSS)and its relationship with disease activity.Methods From May 2017 to September 2019,we select 20 outpatients and inpatients of Qinghai Provincial People's Hospital as the case group,and select 20 healthy people as the control group during the same period.Their peripheral blood mononuclear cells were extracted by density gradient centrifugation method and B lymphocytes are Isolated by magnetic beads sorting method.The relative expression levels of miRNA-7 in B lymphocytes and plasma were detected by reverse transcription-polymerase chain reaction(RT-PCR)in the case group and the control group,and the correlation analysis was used to study the relationship between the expression of miRNA-7 in B lymphocytes and disease activity.Results The miRNA-7 in B lymphocytes of the case group was significantly up-regulated compared with the control group[(0.53±0.17)vs.(0.39±0.11)],and the difference was statistically significant(P<0.05).The expression of miRNA-7 in plasma was not significantly different between the case group(0.41±0.14)and the control group(0.33±0.13)(P>0.05).Compared with plasma,miRNA-7 was significantly up-regulated(0.53±0.17)vs.(0.41±0.14)in B lymphocytes of the case group,and the difference was statistically significant(P<0.05).However,there was no significant correlation between the expression of miRNA-7 in B lymphocytes and plasma(P>0.05).The relative expression level of miRNA-7 was positively correlated with EULAR Sjogren’s synddrome disease activity index(ESSDAI)(r=0.635,P<0.05),Sjogren’s synddrome related antigen B antibody(anti-SSB)(rs=0.490,P<0.05),immunoglobulin G(IgG)(r=0.455,P<0.05),immunoglobulin A(IgA)(r=0.451,P<0.05),Negatively correlated with complement 4(C4)(r=-0.643,P<0.01),white blood cell count(WBC)(rs=-0.489,P<0.05),and had no significant correlation with Sjogren’s synddrome related antigen A antibody(anti-SSA),immunoglobulin M(IgM),C-reaction protein(CRP),erythrocyte sedimentation rate(ERS),platelet count(PLT),helicorubin(Hb),rheumatoid factor(RF),and Anticardiolipin antibodies(ANA).Conclusions miRNA-7 may be involved in the occurrence and development of primary Sjogren's syndrome,and it may become one of the targets for the treatment of primary Sjogren's syndrome in the future,and may also be an indicator of clinical monitoring of disease activity.
