基于数据挖掘的郑承铎主任关于慢性咳嗽临床用药研究

A study on clinical medication of treating chronic cough by director Zheng Chengduo based on data mining

目的:通过数据挖掘结合临床跟诊心得总结郑主任门诊辨证治疗慢性咳嗽的用药规律。方法:对郑承铎主任临床诊治的137例慢性咳嗽病案的处方进行数据化处理,运用Excel表格对样本药物频次、中医证型进行频次统计;运用Clementine12.0进行药物关联规则分析。结果:郑承铎主任临床治疗慢性咳嗽的高频药物为杏仁、陈皮、半夏、茯苓、厚朴、枳实、蜜麻黄、桃仁、黄芪等39味,按功效大致可归化痰止咳平喘药、补虚药、解表药、理气药、化湿药等9类。利用关联规则的药物组方规律分析得出两位药组合12对,两位以上药物组合7组。结论:郑主任在慢性咳嗽的治疗上以“二陈汤”“温胆汤”加味活血化瘀药物等化痰活血治法为根本治法,急性发作时可用“三拗汤”为主的宣肃肺气之法,注重他脏用药。

Objective:Through data mining and clinical follow-up experience,to summarize the medication law of treating chronic cough based on syndrome differentiation from Zheng director’s outpatient.Methods:137 cases of chronic cough cases were processed with data,the frequency of sample drugs and TCM syndrome types were analyzed with Excel,and the drug association rules were analyzed with Clementine12.0.Results:There are 39 kinds of high-frequency drugs for chronic cough,such as Xingren(杏仁),Chenpi(陈皮),Banxia(半夏)and so on.According to the efficacy,these drugs were classified into nine categories:Huatan Zhike Pingchuan(化痰止咳平喘药),Buxu(补虚药),Jiebiao(解表药)and so on.According to the analysis of association rules,there were 12 pairs of two drug combinations and 7 groups of more than two drug combinations.Conclusion:In the treatment of chronic cough,the Huoxue Huayu therapy(活血化瘀法)as the Erchen decoction(二陈汤)and Wendan decoction(温胆汤)was fundamental treatment.When acute attack,the Xuansu Feiqi therapy(宣肃肺气法)as San’ao decoction(三拗汤)can be used,and should be paying attention to the treatment of other organs.
