达比加群酯联合替格瑞洛治疗急性脑梗死并深静脉血栓临床观察

Dapigatron Ester Combined with Tiglillo for Patients with Deep Vein Thrombosis of Acute Cerebral Infarction

目的探讨达比加群酯联合替格瑞洛治疗急性脑梗死并深静脉血栓(DVT)的临床疗效。方法选取医院2017年4月至2019年4月收治的急性脑梗死并DVT患者86例,随机分为观察组和对照组,各43例。两组患者均予急性脑梗死基础治疗及物理疗法,并予替格瑞洛片口服,观察组患者加服达比加群酯胶囊,均持续治疗1周。结果观察组总有效率为90.70%,显著高于对照组的74.42%(P<0.05);治疗后,观察组患者的D-二聚体(D-D)、纤维蛋白原(Fib)、肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)、白细胞介素6(IL-6)及高切全血黏度(HBV)、中切全血黏度(MBV)、低切全血黏度(LBV)水平均显著低于对照组,活化部分凝血活酶时间(APTT)显著长于对照组(P<0.05);观察组与对照组不良反应发生率相当(13.95%比9.30%,χ^2=0.453,P=0.510>0.05)。结论达比加群酯联合替格瑞洛治疗急性脑梗死并DVT,能改善凝血功能、血液流变学及炎性因子水平。

Objective To investigate the clinical efficacy of dapigatron ester combined with tiglillo on serum factor levels in patients with acute cerebral infarction and deep vein thrombosis(DVT).Methods A total of 86 patients with acute cerebral infarction and DVT of lower extremities admitted to the hospital from April 2017 to April 2019 were divided into the observation group and control group,43 cases in each group.Both groupswere treated with basic treatment and physical therapy for acute cerebral infarction,and tegrillo tablets were taken orally.The observation group was treated with dabigatran etexilate capsules.The treatment was continued for 1 week in both groups.Results The total effective rate of the observation group was 90.70%,which was significantly higher than74.42%in the control group(P<0.05).After treatment,the D-dimer(D-D),fibrinogen(Fib),tumor necrosis factor-α(TNF-α),hypersensitivity C reaction Protein(hs-CRP),interleukin-6(IL-6),high-cut whole blood viscosity(HBV),mid-cut whole blood viscosity(MBV),and low-cut whole blood viscosity(LBV)levels in the observation group were significantly lower than those of the control group,and the activated partial thromboplastin time(APTT)was significantly longer than that of the control group(P<0.05);the incidence of adverse reactions in the observation group and the control group was equivalent(13.95%vs.9.30%,χ^2=0.453,P=0.510>0.05).Conclusion Dabigatran etexilate combined with tegrillo in the treatment of acute cerebral infarction with DVT can improve blood coagulation function,blood rheology and inflammatory factor levels.
