家庭医生团队-全专云平台协同管理冠心病PCI术后患者效果分析

Analysis of the effect of collaborative management of patients with coronary heart disease after PCI by the general practice-specialist cloud platform

目的:探讨家庭医生团队-全专云平台协同管理冠心病PCI术后患者的效果。方法:选取辖区内2016年1-12月首次行PCI术后患者127例,随机分为干预组64人和对照组63人。对对照组患者进行常规随访;对干预组患者进行家庭医生签约,家庭医生团队-全专云平台协同管理,并提供家庭医生服务、全专云平台咨询问诊、预约转诊、会诊、协助诊疗等服务。随访时间为2年,比较两组患者的生活质量、满意度评估及年诊疗费用。结果:干预组患者的生活质量量表中的生理机能、一般健康状况、精力、社会功能、精神健康评分高于对照组,差异有统计学意义(P<0.05);干预组男性患者的满意度评分高于对照组,差异有统计学意义(P<0.05);干预组患者在二级医院、三级医院的年诊疗费用及年合计费用均低于对照组,差异有统计学意义(P<0.05)。结论:家庭医生团队-全专云平台协同管理可以提高冠心病PCI术后患者生活质量,提高患者社区就诊满意度,降低诊疗费用。

Objective:To discuss the effect of the general practice-specialist cloud platform for collaborative management of patients with coronary heart disease after percutaneous coronary intervention(PCI).Methods:A total of 127 patients who underwent PCI for the first time from January to December 2016 were selected in the jurisdiction,and randomly divided into an intervention group with 64 cases and a control group with 63 cases.The patients in the control group received conventional follow-up;the patients in the intervention group were signed by family doctors,and managed with the general practice-specialist cloud platform that provided family doctor services,consulting and consultation services on the general practice-specialist cloud platform,appointment referral,consultation and assistance in diagnosis and treatment.The followup time was 2 years,and the quality of life,satisfaction evaluation and annual cost of diagnosis and treatment were compared between the two groups.Results:The scores of physiological function,general health status,energy,social function,and mental health in the quality of life scale in the intervention group were higher than those in the control group,and the difference was statistically significant(P<0.05);the satisfaction scores of male patients in the intervention group were higher than those in the control group,and the difference was statistically significant(P<0.05).The annual diagnosis and treatment expenses and total annual expenses in patients in the intervention group in the secondary and tertiary hospitals were lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:The collaborative management of the general practice-specialist cloud platform can improve the quality of life patients with coronary heart disease after PCI,improve the satisfaction of patients and reduce the cost of diagnosis and treatment.
