超前干预性艾灸对腰椎麻醉下行下肢骨折手术术后患者首次排尿的护理研究

The influence of advanced interventional moxibustion on the postoperative first urination in patients received lower limb surgery under spinal anesthesia

目的观察超前干预性艾灸对腰椎麻醉下行下肢骨科手术后患者首次排尿的影响。方法选择2019年2—9月本院收治的288例腰麻下肢手术患者作为研究对象,按随机数表法分为观察组(147例)及对照组(141例)两组。对照组给予常规护理,观察组在对照组的基础上从术前1天开始行超前干预性艾灸,两组均治疗均5天。观察两组患者治疗有效率、术后首次排尿时间、首次排尿量、尿潴留发生率及SAS评分。结果观察组治疗有效率明显较对照组高(P<0.05)。与对照组比较,观察组首次排尿时间明显缩短、首次排尿量明显增多、尿潴留发生率明显降低(P<0.05)。两组术前1 d、术后2~4 h、术后3 d的SAS评分有统计学意义(P>0.05);两组术后3 d的SAS评分明显较术前1 d及术后2~4 h低,且观察组明显低于对照组(P<0.05)。结论超前干预性艾灸可提高治疗效果,缩短腰麻下肢术后患者首次排尿时间、增加首次排尿量、减少尿潴留发生率,较好地缓解焦虑情绪。

Objective To investigate the effect of advanced interventional moxibustion on the postoperative first urination of patients received lower extremity orthopedic surgery under lumbar spine anesthesia.Methods 288 patients received lower extremity surgery under spinal anesthesia in our hospital from Feb.2019 to Sep.2019 were enrolled as the study subjects,and they were randomly divided into observation group(147)and control group(141),patients in the control group were given routine nursing care,and patients in the observation group were given advanced intervention moxibustion from 1 day before the operation.Both groups of nursing treatments lasted 5 days.The total efficacy,the postoperative first urination time,the first urination volume,the incidence of urinary retention and the SAS score in the two groups were observed and compared.Results The total efficacy in the observation group was higher than control group(P<0.05).Compared with control group,the first urination time in the observation group was significantly shorter,the first urination volume was significantly increased,and the incidence of urinary retention was significantly reduced(P<0.05).There were significant difference between the two groups in the SAS score at the time points one day before operation,2~4 h and 3d after surgery(P<0.05).2~4 h and 3d after surgery,the the SAS score in both groups were significantly lower than that at one day before surgery,and that of observation group was significantly lower than control group(P<0.05).Conclusions Advance interventional moxibustion could improve the efficacy,shorten the postoperative first urination time,increase the first urination volume,and reduce the incidence of urinary retention in patients received lower extremity orthopedic surgery under lumbar spine anesthesia.
