甲氨蝶呤与甲酰四氢叶酸联合治疗宫外孕效果分析

Efficacy of methotrexate combined with formyltetrahydrofolate on ectopic pregnancy

目的探讨甲氨蝶呤(MTX)与甲酰四氢叶酸(CF)联合治疗宫外孕的疗效。方法选取本院2013年6月至2016年2月诊治的92例宫外孕患者,随机分为对照组和研究组各46例。对照组给予MTX治疗,研究组给予MTX与CF联合治疗。观察两组临床疗效、血清β-hCG、FSH水平变化。结果研究组总有效率高于对照组(P<0.05);研究组血β-hCG转阴时间、包块消失时间、腹痛消失时间、阴道出血消失时间均低于对照组(P<0.05);治疗后两组血β-hCG及包块直径均低于治疗前(P<0.05),且研究组低于对照组(P<0.05);两组患者血清卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E 2)水平较治疗前均下降(P<0.05),且研究组低于对照组(P<0.05);研究组不良反应发生率、受精成功率与宫内妊娠率与对照组比较差异无统计学意义(P>0.05)。结论MTX联合CF治疗宫外孕疗效显著,可明显降低血β-hCG水平及FSH水平,有效缩短治疗疗程,且治疗后受精成功率及宫内妊娠率均较高。

Objective To investigate the effect of methotrexate(MTX)combined with formyltetrahydrofolate(CF)in the treatment of ectopic pregnancy.Methods A total of 92 cases ectopic pregnancy diagnosed and treated in our hospital from June 2013 to February 2016 were selected and randomly divided into control group and study group,with 46 cases in each group.The control group was treated with MTX,and the study group was treated with MTX combined with CF,and the clinical effect,serumβ-hCG and FSH levels of the two groups were observed.Results The total effective rate of the study group was higher than that of the control group(P<0.05);the time of bloodβ-hCG turning negative,the time of mass disappearing,the time of abdominal pain disappearing and the time of vaginal bleeding disappearing in the study group were lower than those in the control group(P<0.05);after treatment,the bloodβ-hCG and the diameter of the mass in the two groups were lower than before treatment(P<0.05),and the study group was significantly lower than the control group(P<0.05);the levels of follicle stimulating hormone(FSH),luteinizing hormone(LH)and estradiol(E 2)in the two groups were lower than those before treatment(P<0.05),and the study group was lower than the control group(P<0.05);and there was no significant difference in the incidence of adverse reactions,fertilization success rate and intrauterine pregnancy rate between the study group and the control group(P>0.05).Conclusion MTX combined with CF is effective in the treatment of ectopic pregnancy,which can significantly reduce the levels of bloodβ-hCG and FSH,effectively shorten the treatment course,and the success rate of fertilization and the rate of intrauterine pregnancy are higher after treatment.
