卒中护士预检分诊对卒中救治效率的影响

Influence of Stroke Nurse Pre-triage on Stroke Treatment

目的探讨卒中护士预检分诊对急性卒中救治效率的影响。方法提取院内卒中信息化管理系统自2018年1月至2019年6月急性卒中纳入绿色通道救治的患者1968例的信息;根据是否存在二次分诊,分为研究组(存在二次分诊,453例)和对照组(1515例)。回顾性分析卒中护士预检分诊情况,并对二次分诊原因进行分析。结果研究组到院至分诊完成、分诊完成至建立静脉通道较对照组明显延长,差异有统计学意义(均P<0.05);而两组在调整的分诊完成至建立通道时间方面的差异无统计学意义(P>0.05)。导致二次分诊的原因包括卒中护士判定为非卒中和等待卒中医生最终判定等。结论卒中护士预检分诊的准确性及效率会影响卒中患者救治时间;绿色通道仍需要持续优化。

Objective To explore the influence of stroke nurse pre-triage on the efficacy of acute stroke treatment.Methods Data of 1968 cases diagnosed with acute stroke and enrolled in rapid stroke channel from January 2018 to June 2019 were extracted from the information management system and analyzed.The cases were divided into two groups based on secondary triage or not,including study group(with secondary triage,453 cases)and control group(without secondary triage,1515 cases).The influence of stroke nurse pre-triage on performing rapid stroke channel was compared between the two groups,and the reasons for secondary triage were explored.Results Compared with the control group,the median time of door-to-triage completed and triage completed-to-intravenous line were significantly longer in the study group(all P<0.05).No significant differences were noted between the two groups with regard to median time of adjusted triage completed-to-intravenous line(P>0.05).Pre-diagnosed as non-stroke and waiting for stroke doctors were the major reasons for secondary triage.Conclusions The stroke treatment time would be affected by the accuracy and efficiency of stroke nurse pre-triage.The rapid stroke channel needs to be optimized continuously.
