基于肠肾关系探讨从胃肠论治IgA肾病

Discussion on Treatment of IgA Nephropathy from Gastrointestinal Perspective Based on Intestinal-Renal Relationship

IgA肾病多与炎症性肠病、肠道菌群紊乱、肠道黏膜通透性增加所致的胃肠道黏膜免疫缺陷有关。胃肠道黏膜系统的完整多与中医胃主腐熟、肠主传化的功能有关。本文基于中医胃肠与肾的关系,探讨通过健脾和胃、通腑行滞、清利湿热、调节气机等法清利在胃腑之湿热浊毒等实邪,以达对IgA肾病干预的目的,"通利胃肠以安脏补虚"为IgA肾病的临证治疗提供了新的思路和方法。

IgA nephropathy is often associated with gastrointestinal mucosal immunodeficiency caused by inflammatory bowel disease,intestinal flora disorder and increased intestinal mucosal permeability.The integrity of the gastrointestinal mucosa system is mostly related to the function of stomach dominating maturity and intestine dominating transmission in TCM.Based on the relationship between gastrointestinal tract and kidney in TCM,this article explored the intervention of IgA nephropathy by the method of clearing away dampness heat turbid poison and other evils in the stomach,such as invigorating spleen and stomach,dredging the internal organs and promoting stagnation,clearing away dampness and heat,regulating qi movement,etc."Clearing the gastrointestinal tract for the purpose of tonifying the deficiency of internal organs"provides a new idea and method for the clinical treatment of IgA nephropathy.
