口炎清颗粒联合奥硝唑对慢性牙周炎患者高迁移率族蛋白B1、肿瘤坏死因子α、白细胞介素17和白细胞介素23表达的影响

Effect of Kouyanqing granule combined with ornidazole on the expression of HMGB1,TNF-α,I L-17 and IL-23 in patients with chronic periodontitis

目的 探讨口炎清颗粒联合奥硝唑对慢性牙周炎患者高迁移率族蛋白B1(HMGB1)、肿瘤坏死因子α(TNF-α)、白细胞介素17 (IL-17)和白细胞介素23(IL-23)表达的影响.方法 选择杭州市萧山区第一人民医院2018年10月至2019年10月收治的慢性牙周炎患者162例,依据随机数字表法分为观察组和对照组各81例;对照组患者口服奥硝唑治疗,观察组在对照组基础上口服口炎清颗粒治疗.两组疗程均为4周.比较两组治疗疗效、治疗前后菌斑指数(PLI)、牙周袋探诊深度(PD)、出血指数(BI)、HMGB1、TNF-α、IL-17和IL-23水平变化及不良反应情况.结果 观察组总有效率(91.36%)高于对照组(77.78%)(x2=5.723,P<0.05);观察组治疗后PLI(0.93 ±0.16)、PD[(3.15 ±0.37) mm]和BI(1.35 ±0.24)均低于对照组[(1.42±0.24)、(4.28 ±0.45) mm和(2.18±0.28)](t=15.289、17.457、20.256,均P<0.05);观察组治疗后HMGB1[(0.78±0.13) μg/L]、TNF-α[(3.10±0.32) μg/L]、IL-17 [(368.19±24.31) ng/L]和IL-23[(7.63±0.89) ng/L]均低于对照组[(0.96±0.10) μg/L、(4.34±0.46) μg/L、(457.32±21.35) ng/L和(10.32±1.25) ng/L](t=9.877、19.916、24.793、15.777,均P<0.05);观察组不良反应发生率(6.17%)低于对照组(17.28%)(x2 =4.830,P<0.05).结论 口炎清颗粒联合奥硝唑对慢性牙周炎患者疗效明显,可下调HMGB1、TNF-α、IL-17和IL-23水平,且不良反应少.

Objective To investigate the efeet of Kouyanqing granule combined with oridazole on the expression of high mobility group protein BI(HMCB1),tumor necrosis factor-α(TNF-α),interleukin-17(IL-17)and interleukin-23(IL-23)in patients with chronie periodontitis.Methods From October 2018 to October 2019,162 patients with chronic periodontitis admitted to the First Peoples Hospital of Xiaoshan District were divided into observation group(81 cases)and control group(81 cases)according to the random digital table method.The control group was treated with ornidazole,and the observation group was treated with Kouyanqing granules on the basis of the control group.The course of treatment in both two groups was 4 weeks.The changes of plaque index(PLI),probing depth of periodontal pocket(PD),bleeding index(BI),HMGBI,TNF-α,IL-17 and IL-23 and adverse reactions were compared before and after treatment.Results The total ffctive rate of the observation group(91.36%)was higher than that of the control group(77.78%)(χ2=5.723,P<0.05).The PLI(0.93±0.16),PD[(3.15±0.37)mm]and BI(1.35±0.24)in the observation group were lower than those in the control group[(1.42±0.24),(4.28±0.45)mm and(2.18±0.28)](t=15.289,17.457,20.256,all P<0.05).The levels of HMCBI[(0.78±0.13)μg/L],TNF-α[(3.10±0.32)μg/L],IL-17[(368.19±24.31)ng/L]and IL-23[(7.63±0.89)ng/L]in the observation group were lower than those in the control group[(0.96±0.10)ug/L,(4.34±0.46)μg/L,(457.32±21.35)ng/L and(10.32±1.25)ng/L](t=9.877,19.916,24.793,15.77,all P<0.05).The incidence of adverse reactions in the observation group(6.17%)was lower than that in the control group(17.28%)(χ2=4.830,P<0.05).Conclusion Kouyanqing granule combined with ornidazole is efective in the treatment of chronic periodontitis.It can reduce the levels of HMCB1,TNF-α,IL-17 and IL-23,and has few adverse reactions.
