米非司酮与左炔诺孕酮宫内节育系统治疗围绝经期功能失调性子宫出血的效果

Effect of mifepristone and levonorgestrel intrauterine system in the treatment of perimenopausal dysfunctional uterine bleeding

目的观察米非司酮与左炔诺孕酮宫内节育系统治疗围绝经期功能失调性子宫出血的效果。方法选取我院2017年1月至2019年9月收治的76例围绝经期功能失调性子宫出血患者作为研究对象,将其随机分为对照组和观察组,各38例。对照组给予米非司酮治疗,观察组给予左炔诺孕酮宫内节育系统治疗。比较两组的治疗效果。结果治疗1个月、治疗3个月及治疗后3个月,观察组的PBAC评分、子宫内膜厚度及血红蛋白水平均优于对照组(P<0.05)。观察组的不良反应总发生率低于对照组,治疗总有效率高于对照组(P<0.05)。治疗后3个月,两组的SF-36评分均升高,且观察组高于对照组(P<0.05)。结论相较于米非司酮,左炔诺孕酮宫内节育系统治疗围绝经期功能失调性子宫出血的效果更优,能有效控制控制患者异常出血,改善子宫内膜厚度及贫血状况,且不良反应发生率较低,值得临床推广应用。

Objective To observe the effect of mifepristone and levonorgestrel intrauterine system in the treatment of perimenopausal dysfunctional uterine bleeding.Methods A total of 76 patients with perimenopausal dysfunctional uterine bleeding treated in our hospital from January 2017 to September 2019 were selected as the research objects,and the patients were randomly divided into control group and observation group,with 38 cases in each group.The control group was treated with mifepristone,and the observation group was treated with levonorgestrel intrauterine system.The therapeutic effects of the two groups were compared.Results At 1 month of treatment,3 months of treatment and 3 months after treatment,the PBAC scores,endometrial thickness and hemoglobin levels in the observation group were better than those in the control group(P<0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group,and the total effective rate of treatment was higher than that in the control group(P<0.05).Three months after treatment,the SF-36 scores of both groups increased,and those in the observation group were higher than the control group(P<0.05).Conclusion Compared with mifepristone,the levonorgestrel intrauterine system has better effect in the treatment of perimenopausal dysfunctional uterine bleeding,it can effectively control abnormal bleeding in patients and improve endometrial thickness and anemia,which has low incidence of adverse reactions and is worthy of clinical popularization and application.
