补虚涤痰汤治疗老年气虚痰阻型椎-基底动脉供血不足性眩晕临床研究

Clinical Study of Buxu Ditan Decoction for Treatment of Vertebro-basilar Artery Insufficiency Vertigo with Qi Deficiency and Phlegm Stasis in the Aged

目的观察补虚涤痰汤治疗老年气虚痰阻型椎-基底动脉供血不足性眩晕的临床疗效。方法将98例椎-基底动脉供血不足性眩晕患者随机分为对照组和观察组,每组49例。2组均给予尼莫地平治疗,在此基础上,观察组加用补虚涤痰汤治疗。2组均治疗1个月。统计2组治疗后临床疗效,比较2组左椎动脉(left vertebral artery,LVA)、右椎动脉(right vertebral artery,RVA)、基底动脉(basilar artery,BA)的平均血流速度及眩晕症状评分、生活质量评分、焦虑评分、抑郁评分等指标。结果治疗后,观察组总有效率为95.92%,显著高于对照组的83.67%(P<0.05)。2组LVA、RVA、BA平均血流速度均较治疗前提高(P<0.05),且观察组上述指标均高于对照组(P<0.05)。2组VSS-C评分均低于治疗前(P<0.05),SF-36评分均高于治疗前(P<0.05);且观察组VSS-C评分低于对照组(P<0.05),SF-36评分高于对照组(P<0.05)。2组HAMA评分及HAMD评分均低于治疗前(P<0.05),且观察组上述评分均低于对照组(P<0.05)。结论补虚涤痰汤治疗老年气虚痰阻型椎-基底动脉供血不足性眩晕疗效显著,其能加快患者脑部血流速度,改善眩晕症状,解除焦虑、抑郁等不良情绪,提高生活质量。

Objective To observe the clinical efficacy of Buxu Ditan decoction in the treatment of senile vertebro-basilar artery insufficiency vertigo with qi deficiency and phlegm stasis.Methods A total of 98 patients with vertebro-basilar artery insufficiency vertigo were randomly divided into control group and observation group,with 49 cases in each group.Both groups were treated with nimodipine.On this basis,the observation group was treated with Buxu Ditan decoction.Both groups were treated for one month.The clinical efficacy,mean blood flow velocity of left vertebral artery(LVA),right vertebral artery(RVA)and basilar artery(BA),vertigo symptom scores,quality of life scores,anxiety scores and depression scores were compared in both groups.Results After treatment,the total effective rate of the observation group was 95.92%,which was higher than that of the control group(P<0.05).The mean blood flow velocity of LVA,RVA and BA in the two groups were all higher than those before treatment(P<0.05),and those in the observation group were higher than the control group(P<0.05).The VSS-C scores of the two groups were lower than those before treatment(P<0.05),while the SF-36 scores were higher than those before treatment(P<0.05).The VSS-C scores of observation group were lower than those of control group(P<0.05),and the SF-36 scores were higher than the control group(P<0.05).The HAMA and HAMD scores in the two groups were all lower than those before treatment(P<0.05),and the above scores in the observation group were lower than those in the control group(P<0.05).Conclusion Buxu Ditan decoction has a remarkable effect on the treatment of vertebro-basilar artery insufficiency vertigo with qi deficiency and phlegm stasis in the elderly.It can speed up the blood flow of the brain,improve the symptoms of vertigo,relieve anxiety,depression and other adverse emotions,and improve the quality of life.
