广西公立三甲医院疾病与健康管理模式效果研究

Research on Effect of Disease and Health Management Model in Guangxi Public 3A Hospitals

目的评价广西公立三甲医院疾病与健康管理服务的效果。方法采用整群抽样方法,抽取2017年7月至2019年6月到广西医科大学第二附属医院内分泌科、神经内科、普通外科、产科就诊的患者为调查对象。利用健康管理系统上已有的、患者对医疗服务的满意问卷进行线上调查;查阅医院健康管理系统和医院信息系统数据库,收集开展疾病与健康管理服务前后相关的数据资料。结果开展疾病与健康管理工作后的随访率显著高于开展疾病与健康管理工作前[82.44%(587/712)比53.47%(285/533)](P<0.01)。同时段患者复诊率比较,开展疾病管理服务后4个试点科室(内分泌科、神经内科、普通外科、产科)患者1次复诊率及2次复诊率均高于未开展疾病管理服务前。结论开展疾病与健康管理工作不仅有助于提高医疗服务水平,更有利于患者尤其是慢性病患者恢复健康,确保其获得正确指导,纠正不良行为、提高依从性。

Objective To evaluate the effect of disease and health management model in Guangxi public 3A hospitals.Methods Cluster sampling method adopted,we collected cases in the Second Affiliated Hospital of Guangxi Medical University,from Jul.2017 to Jun.2019,including patients from the clinics of endocrinology,neurology,general surgery,and maternity as the investigation objects.Medical service satisfaction questionnaire online survey was made by the health mana-gement system.The hospital health management system and hospital information system database was consulted,to collect the relevant data of before and after disease and the health management service.Results The follow-up rate after carrying out disease and health management was significantly higher than before[82.44%(587/712)vs 53.47%(285/533)](P<0.01).During the same period,the rates of 1 revisit and 2 revisits in the four pilot departments(Endocrinology,Neurology,General Surgery,Obstetrics)were higher than before the disease management service.Conclusion The implementation of disease management service is helpful not only to improve medical services,but also to the recovery of patients,which can make ensure they can receive the correct guidance,correct bad behaviors,and improve patient compliance.
