摘要
目的探究胶质瘤磁共振弥散张量成像(DTI)量化参数相对表观弥漫系数(relative apparent diffusion coefficient,rADC),相对各向异性分数(relative fractional anisotropy,rFA)与Ki-67,p53表达程度的相关性。方法回顾性分析30例经术后病理证实的胶质瘤患者的病历资料,术前行常规MR平扫、增强、DTI扫描,术后行Ki-67,p53表达程度免疫组化检测,对高、低级别组肿瘤实质rADC、rFA值进行比较并分析胶质瘤rADC,rFA值分别与Ki-67,p53表达程度之间的关系。结果高、低级别组胶质瘤肿瘤实质rADC值分别为1.508±0.348,1.980±0.425,差异具有统计学意义(P<0.05);高、低级别组胶质瘤肿瘤实质rFA值分别为0.407±0.067,0.315±0.135,差异具有统计学意义(P<0.05);胶质瘤肿瘤实质rADC值分别与Ki-67,p53表达呈负相关(rs值分别为-0.686,-0.387,P<0.05),胶质瘤肿瘤实质rFA值与分别Ki-67,p53表达程度呈正相关(rs值分别为0.370,0.361,P<0.05)。结论DTI量化参数rADC,rFA值可以在一定程度上反映肿瘤实质Ki-67,p53表达水平,从而间接评估胶质瘤恶性程度及其增殖情况。
Objective To investigate the correlation between the quantitative parameters rADC,rFA and the expression of Ki-67 and p53 in glioma MRI diffusion tensor imaging(DTI).Methods A retrospective analysis of 30 patients with glioma confirmed by postoperative pathology was performed.Routine MR scans,enhanced scans,and DTI scans were performed before operation,and Ki-67 and p53 expression levels were detected by immunohistochemistry after operation.The tumor parenchymal rADC and rFA values of the high and low grade groups were compared and the relationship between the glioma DTI quantitative parameters rADC and rFA values and the expression of Ki-67 and p53 was analyzed.Results The rADC values of glioma tumor parenchyma in the high-grade and low-grade groups were 1.508±0.348 and 1.980±0.425,respectively,and the difference was statistically significant(P<0.05).The parenchymal rFA values of glioma tumors in the high-grade and low-grade groups were 0.407±0.067 and 0.315±0.135,respectively,and the difference was statistically significant(P<0.05).The parenchymal rADC value of glioma tumors was negatively correlated with the expression of Ki-67 and p53(rs were-0.686,-0.387,P<0.05),which was positively correlated with the expression levels of Ki-67 and p53(rs were 0.370,0.361,P<0.05).Conclusion The rADC and rFA of DTI quantification parameters can reflect the expression levels of Ki-67 and p53 in tumor parenchyma to a certain extent,thereby indirectly assessing the malignant degree and proliferation of glioma.
