糖尿病患者延续性护理的研究进展

Research progress of continuous care for diabetic patients

糖尿病病情复杂且为终身性疾病,延续性护理是院内护理行为至家庭或社区的延伸,在糖尿病管理中起重要作用。与国外相比,我国延续性护理起步较晚,发展较慢,在干预方式、护理实施等方面还存在许多问题。文章从延续性护理的概念,糖尿病患者延续性护理的需求以及糖尿病患者延续性护理的实施模式展开综述,依据我国现阶段的实际情况,分析延续性护理存在的一些问题以及对问题的思考,以期为糖尿病患者开展延续性护理提供参考。

Diabetes is complicated and a lifelong disease.Continuous care is an extension of hospital care behavior to the family or community,and it plays an important role in diabetes management.Compared with foreign countries,continuous nursing in our country started late and developed slowly,and there are still many problems in intervention methods and nursing implementation.This article summarizes the concept of continuation care,the needs and the implementation mode of continuous care for the patients with diabetes mellitus.According to the actual in our country this stage,some problems in continuous care and their thinking are analyzed,in order to provide reference for diabetic patients to carry out continuous care.
