通过系统介绍王庆国教授治疗口腔溃疡的诊疗思路和临证经验。王教授认为口腔溃疡的病因主要分为外感风热、饮食不节、情志不畅、劳倦过度及先天禀赋不足五个方面,五种病因皆可致火热内蕴,进而循经上炎,熏蒸口舌,发为口疮。因此,火热内蕴为口腔溃疡发病的根本病机,泻火坚阴为临证相应之根本治法。临证辨证论治时,根据病因不同,火热内蕴之病机又可分为实火和虚火:实火为外感风热、饮食不节所致,对应病机为外感风热、心脾积热,治疗当清热散邪,泻火坚阴;虚火为情志不畅、劳倦过度及先天禀赋不足,对应病机为脾胃虚弱、阴虚火旺和阳虚火炎,治疗上当调补阴阳,泻火坚阴。总而言之,临证应把握根本病机,根据病机辨证分型,灵活诊治,方能提高疗效。通过系统总结王庆国教授辨治口腔溃疡诊疗思路和临证经验,明晰了口腔溃疡的病因、病机和治法,明确了临证治疗思路,以期对临床有所裨益,对后学者有所启示。

This thesis systematically introduces Professor Wang Qingguo's diagnostic ideas and clinical experience in treating oral ulcers.Professor Wang believes that the etiology of oral ulcers is mainly divided into five aspects:exogenous wind and heat,unhealthy diet,poor mood,overwork,and inadequate endowment,all five causes can cause fiery content,which in turn causes menstrual inflammation,fumigation of the tongue,and aphthous ulcers.Therefore,fiery content is the fundamental pathogenesis of oral ulcers,and purging fire and strengthening Yin is the corresponding fundamental cure for clinical evidence.In the clinical diagnosis and treatment,according to the different causes,the pathogenesis of fiery heat can be divided into Shihuo and Xuhuo(实火和虚火).Shihuo is caused by exogenous wind-heat and inadequate diet.The corresponding pathogenesis is exogenous wind-heat and heart and spleen fever,treatment focuses on clearing heat and dispersing evil,purging fire and strengthening Yin.Xuhuo are due to poor mood,overwork,and innate endowment.The corresponding pathogenesis is weak spleen and stomach,Yin deficiency and fire,and Yang deficiency and fire,treatment focuses on Yin and Yang,purging fire and strengthening Yin.All in all,the clinical path should grasp the underlying pathogenesis,identify the type according to the pathogenesis,and flexibly diagnose and treat to improve the efficacy.This article systematically summarizes Professor Wang Qingguo's diagnosis and treatment ideas and clinical experience in the treatment of oral ulcers,clarifies the etiology,pathogenesis and treatment methods of oral ulcers,and elucidates the clinical treatment ideas,with a view to benefiting the clinic and enlightening postscholars.
