红花黄色素联合耳后注射甲强龙治疗突发性耳聋的临床疗效及对免疫功能的影响

Clinical effect of carthamin yellow combined with post-auricular injection of methylprednisolone in the treatment of sudden deafness and its effect on immune function

目的探讨红花黄色素联合耳后注射甲强龙治疗突发性耳聋的临床疗效及对细胞免疫功能的影响。方法选取2017年10月~2019年4月我院收治的120例突发性耳聋患者,按照随机数字法随机分为观察组和对照组,每组各60例。对照组给予耳后局部注射甲泼尼龙琥珀酸针40 mg,观察组在对照组基础上给予红花黄色素100 mg治疗,两组均治疗10 d。观察两组治疗前后纯音听阈(PTA)的变化、血液流变学指标的变化,并对两组治疗前后的C反应蛋白(CRP)、一氧化氮(NO)及细胞免疫指标(CD8^+、CD4^+、CD4^+/CD8^+)进行检测。评估两组患者的临床疗效。结果治疗后两组PTA均显著下降(P<0.05),但是观察组下降更为明显(P<0.05)。治疗后两组血液流变学指标均显著改善(P<0.05),但观察组改善更为明显(P<0.05)。治疗后两组CRP均显著下降,NO均显著上升(P<0.05),但观察组CRP下降更为明显(P<0.05),NO上升更为明显(P<0.05)。与治疗前相比,两组治疗后细胞亚群水平均显著改善(P<0.05),但观察组的细胞亚群水平改善更为明显(P<0.05)。观察组总治疗有效率为90.00%,显著高于对照组的78.33%(P<0.05)。结论红花黄色素联合耳后注射甲强龙治疗突发性耳聋的临床疗效好,可显著改善患者听力水平和血液流变学指标,其作用机制可能与改善细胞免疫学指标有关。

Objective To investigate the clinical effect of carthamin yellow combined with post-auricular injection of methylprednisolone in the treatment of sudden deafness and its effect on cellular immune function.Methods A total of 120 patients with sudden deafness who were admitted to our hospital from October 2017 to April 2019 were selected.They were randomly divided into observation group and control group,with 60 cases in each group.According to the random number method,the control group was given topical post-auricular injection of 40 mg methylprednisolone succinate.The observation group was given 100 mg carthamin yellow on the basis of the control group.Both groups were treated for 10 days.The changes of pure tone audiometry(PTA)and hemorheological indices before and after treatment were observed in both groups.The C reactive protein(CRP),nitric oxide(NO),and cellular immune indicators(CD8^+,CD4^+,CD4^+/CD8^+)were detected before and after treatment in both groups.The clinical efficacy was evaluated in both groups of patients.Results After treatment,the PTA in both groups was decreased significantly(P<0.05),but the decrease in the observation group was more significant(P<0.05).After treatment,the hemorheological indices in both groups were significantly improved(P<0.05),but the improvement in the observation group was more significant(P<0.05).After treatment,CRP was decreased significantly in both groups,and no was significantly increased(P<0.05).However,the CRP in the observation group was decreased more significantly(P<0.05),and the no was increased more significantly(P<0.05).Compared with those before treatment,the levels of cell subsets in both groups were significantly improved after treatment(P<0.05).However,the levels of cell subsets in the observation group were improved significantly(P<0.05).The total effective rate of treatment in the observation group was 90.00%,which was significantly higher than that of 78.33%in the control group(P<0.05).Conclusion Carthamin yellow combined with post-auricular injection of methylprednisolone in the treatment of sudden deafness has a favorable clinical effect,which can significantly improve patients'hearing level and hemorheological indices.Its mechanism of action may be related to the improvement of cellular immunological indicators.
