抑郁症脑网络的静息态功能磁共振分析 被引量:4

摘要 目的分析抑郁症患者脑网络的静息态功能磁共振(rs-fMRI)特征,并评价其与抑郁严重程度的相关性。方法选取在我院心理门诊治疗的抑郁症患者25例作为研究组,同期健康志愿者25例作为对照组,均进行rs-fMRI扫描。并采用Pearson分析有统计学意义的脑区内平均ReHo值与汉密尔顿抑郁量表(HAMD-17)评分的相关性。结果研究组左侧海马、左侧直回、左侧顶下缘角回、右侧楔前叶、右侧顶上回、右侧楔叶的ReHo值明显高于对照组的ReHo值(P<0.05);研究组左侧枕下回、左侧颞下回、右侧颞上回、右侧颞下回、右侧背外侧前额叶、右侧后扣带回的ReHo值明显低于对照组的ReHo值(P<0.05)。经相关性分析,抑郁症患者的HAMD-17评分与左侧海马的ReHo值呈正相关(P<0.001),抑郁症患者的HAMD-17评分与右侧后扣带回的ReHo值呈负相关(P<0.05)。结论抑郁症患者静息态下脑功能损伤主要集中在默认网络和边缘系统等,且左侧海马、右侧后扣带回局部脑区连接强度变化与抑郁症严重程度具有相关性,ReHo可辅助评价抑郁症严重程度。 Objective To analyze the characteristics of resting state functional magnetic resonance imaging(rs-fMRI)in the brain network of patients with depression,and evaluate its correlation with the severity of depression.Methods A total of 25 patients with depression who were treated in the psychological clinic of our hospital were selected as the study group.During the same period,25 healthy volunteers were selected as the control group.All subjects were given rs-fMRI scans.The correlation between the statistically significant average ReHo value in the brain region and the Hamilton Depression Scale(HAMD-17)score was analyzed by Pearson.Results The ReHo values of the left hippocampus,left straight gyrus,left top parietal corner gyrus,right anterior wedge,right top parietal gyrus and right wedge were significantly higher in the study group than those in the control group(P<0.05);ReHo values of the left inferior occipital gyrus,left inferior temporal gyrus,right superior temporal gyrus,right inferior temporal gyrus,right lateral dorsal prefrontal lobe and right posterior cingulate gyrus were significantly lower in the study group than those in the control group(P<0.05).Correlation analysis showed that the HAMD-17 score in depression patients was positively correlated with the ReHo value of the left hippocampus(P<0.001).The HAMD-17 score in depression patients was negatively correlated with the ReHo value of the right posterior cingulate gyrus(P<0.05).Conclusion The resting state brain function injury of patients with depression is mainly concentrated in the default network and limbic system.Moreover,the changes in the intensity of the connection of the local brain area in the left hippocampus and the right posterior cingulate gyrus are correlated with the severity of depression.ReHo can help assess the severity of depression.
