超声引导下腹腔镜子宫肌瘤剔除术的价值研究

目的探讨腹腔镜多发性子宫肌瘤剔除术,术中利用超声引导剔除肉眼不可见的肌壁间肌瘤和黏膜下肌瘤,减少残留,降低复发几率的临床价值。方法2018年1月—2019年2月我院收治的多发性子宫肌瘤患者120例作为研究对象,采用随机方法分为观察组和对照组,每组患者60例。对照组患者进行常规的腹腔镜下子宫肌瘤剔除术,观察组采用超声引导下腹腔镜多发性子宫肌瘤剔除术,比较两组患者的手术时间、术中出血量、肌瘤剔除个数、术后住院时间及术后肌瘤复发情况。结果两组患者手术时间、术中出血量、术后住院时间进行比较,差异无统计学意义(P>0.05),观察组患者子宫肌瘤剔除个数大于对照组(P<0.05)。术后肌瘤复发明显少于对照组(P<0.05)。结论临床上对于多发性子宫肌瘤进行治疗时,为要求保留生育功能患者选择超声引导下腹腔镜子宫肌瘤剔除术,术中利用超声引导可有效的剔除肉眼不可辨别的肌壁间肌瘤及黏膜下肌瘤,减少肌瘤残留,切断根源,从而降低复发几率。

Objective To explore the clinical value of laparoscopic multiple uterine fibroid serofima culling,using ultrasound to guide the removal of intermyal fibroids and mucosa fibroids that are not visible to the naked eye,to reduce residue and reduce the probability of recurrence.Methods From January 2018 to February 2019,120 patients with multiple uterine fibroids were treated in our hospital as a subject,with a random approach divided into observation and control groups of 60 patients per group.The patients in the control group carried out routine uterine fibroid culling under laparoscopy,and the observation group used ultrasound-guided laparoscopic multiple uterine fibroid culling,which compared the surgical time,intraoperative hemorrhage,fibroid culling number,postoperative hospitalization time and postoperative fibroid recurrence.Results The difference was not significant in the two groups of patients’surgery time,intraoperative hemorrhage and postoperative hospitalization time(P>0.05),and the number of uterine fibroid culls in the observation group was greater than that of the control group(P<0.05).Postoperative fibroid recurrence was significantly less than in the control group(P<0.05).Conclusion Clinically for the treatment of multiple uterine fibroids,in order to require patients with reproductive function to choose ultrasoundguided laparoscopic uterine fibroid culling,the use of ultrasound guidance can effectively remove the naked eye can not distinguish between myomaandoma and mucosa fibroids,reduce fibroid residue,cut off the root cause,thereby reducing the chance of recurrence.
