“翻煎饼”结合三位一体手术排台在手术配合中的效率研究

目的观察"翻煎饼"结合三位一体手术排台规则在人工关节置换手术的时间管理中的运用效果,分析在优化手术室工作流程,提高工作效率,加快手术间的周转,节约医疗资源等方面的成效。方法选择2019年6—12月我院收治的120例行人工关节置换手术的患者作为研究对象,随机分为常规排台组及"翻煎饼"结合三位一体排台组(以下简称"翻煎饼"组),每组60例;观察患者从进入手术室到第二个患者入手术室所有节点进行详细的记录,配合监控录像分析,比较两组护理配合准备质量、主刀平均每台手术时间、术前准备时间、患者平均每台等待时间、手术团队满意度调查。结果(1)护理配合准备质量两组对比差异无统计学意义(P>0.05),两组质量均为优。两组质量均为优。(2)从首台到结束时间进行统计,"翻煎饼"组手术主刀在手术室所耗总时间为2186.0分钟,主刀平均每台手术时间为(36.4±7.9)分钟,常规组手术主刀在手术室所耗时间5418.0分钟,主刀平均每台手术时间为(90.3±13.2)分钟,两组比较差异均具有统计学意义(P<0.05)。(3)常规组术前准备时间平均为(41.6±5.9)分钟,"翻煎饼"组(在无菌间开器械台准备物品)术前准备时间平均为(10.5±2.7)分钟,两组比较差异具有统计学意义(P<0.05)。(4)常规组患者等待总时间1550分钟,平均每台等待时间为(25.8±8.0)分钟,"翻煎饼"组患者等待总时间为330分钟,平均每台等待时间为(5.5±1.9)分钟,两组比较差异具有统计学意义(P<0.05)。(5)手术团队满意度包括患者满意率、医生满意率、麻醉师满意率、护士满意率"翻煎饼"组均高于常规组,差异具有统计学意义(P<0.05)。结论在手术室的时间管理中,以"高效团队"理念为指导,采取"翻煎饼"结合三位一体手术排台规则,优化术前准备医护人员的配合流程,术前准备时间、手术总时间、主刀医生手术耗时及接台手术患者等待时间均缩短,提高了工作效率,加快手术间的周转,节省工作时间及医院成本,手术团队整体满意率提高。

Objective The applying effect of"Flipping Pancake"combined with trinity operation table rules in the time management of joint replacement surgery was observed,to analyze the effect on optimizing work processes of the operating room,improving work efficiency,speeding up the operation,saving medical resources.Methods 120 patients who underwent artificial joint replacement surgery between June 2019 and December 2019 were chosed and randomly divided into conventional drainage group(60 cases)and"Flipping Pancake"combined with trinity operation table group(60 cases).According to detailed records of all the nodes from a patient entering the operating room until the other patient entering and video analysis,preparing quality of nursing cooperate,the average surgeon operation time,preoperative preparation time,average waiting time of each patient,surgical team satisfaction were compared between two groups.Results(1)Prepare quality of nursing cooperate of two groups are good,showing no significant differences(P>0.05).(2)From the first to the end of time,the surgeon of"Flipping Pancake"combined with trinity operation table group in the operating room by a total of 2186.0 minutes,average operation time for each work(36.4±7.9)minutes,which is less than that of conventional drainage group by a total of 5418.0 minutes,average operation time for each work(90.3±13.2)minutes,showing significant differences(P<0.05).(3)Preoperative preparation time of"Flipping Pancake"combined with trinity operation table group is(10.5±2.7)minutes,which is less than that of conventional drainage group(41.6±5.9)minutes,showing significant differences(P<0.05).(4)Average waiting time of each patient of"Flipping Pancake"combined with trinity operation table group is(5.5±1.9)minutes,which is less than that of conventional drainage group(25.8±8.0)minutes,showing significant differences(P<0.05).(5)The satisfaction of the surgical team including doctor,nurse and anesthesiologist of"Flipping Pancake"combined with trinity operation table group is better than that of conventional drainage group,showing significant differences(P<0.05).Conclusion Guided by the"high efficiency team"concept,taking"Flipping Pancake"combined with trinity operation table rules in the operating room time management can optimize preoperative preparation,and reduce average surgeon operation time and average waiting time of each patient to improve work efficiency,speed up the operation,save the working time and hospital costs,and increase the surgical team overall satisfaction rate.
