补肾益肝通络方联合针刀治疗后循环缺血性眩晕临床应用价值分析

Clinical application value of the Bushen Yigan Tongluo prescription plus acupotomy on posterior circulation ischemic vertigo

目的:运用传统医学方法,探索补肾益肝通络方联合针刀技术对后循环缺血性眩晕(Posterior Circulation Ischemic Vertigo,PCIV)治疗效果及推广应用价值。方法:随机将80例临床确诊为后循环缺血性眩晕患者分为观察组和对照组,每组各40例。对照组采用西医常规治疗,观察组采用补肾益肝通络方联合针刀技术,观察两组患者治疗期间各阶段欧洲头晕评估量表(The European Evaluation of Vertigo Scale,EEV)、眩晕等级评定量表的评分系统(DARS)、经颅超声多普勒(Transcranial Doppler,TCD)血管检测情况,并评价两组疗效及安全性。结果:经治疗1 d后,观察组在改善眩晕症状、提高血流速度方面明显优于对照组,差异有统计学意义(P<0.01)。治疗2个疗程后,两组均可有效降低EEV评分和DARS评分,改善TCD检测结果,观察组疗效明显优于对照组,两组组间和组内比较差异均有统计学意义(P<0.05)。2个疗程结束后与治疗1个月以后进行EEV、DARS评分和TCD检测指标分析,两组内比较,差异均无统计学意义(P>0.05),说明远期疗效稳定。结论:经补肾益肝通络方联合针刀治疗后,患者头晕症状得到有效缓解,椎动脉和基底动脉的平均血流速度明显改善,且安全性高,中长期疗效稳定,值得在临床中推广应用。

Objective:To explore the therapeutic effect of the Bushen Yigan Tongluo prescription(补肾益肝通络方)plus acupotomy on posterior circulation ischemic vertigo and its application value by traditional medical methods.Methods:80 patients were randomly divided into the observation group and control group.The control group was treated with conventional Western medicine,while the observation group was treated with the Bushen Yigan Tongluo prescription plus acupotomy.Results:After one day of treatment,the observation group was significantly superior to the control group in improving vertigo symptoms and blood flow velocity(P<0.01).After two courses of treatment,both groups could effectively reduce EEV score and DARS score,and improve TCD test results.The efficacy in the observation group was significantly better,with statistically significant differences(P<0.05).EEV,DARS score and TCD test indexes were analyzed after the completion of 2 courses of treatment and one month after treatment.No statistically significant difference was found(P>0.05),indicating stable long-term efficacy.Conclusion:After the treatment of the Bushen Yigan Tongluo prescription plus acupotomy,the dizziness symptoms of the patients can be effectively relieved,and the average blood flow velocity of the vertebral artery and basilar artery can be significantly improved.
