摘要
目的:探讨社区2型糖尿病患者趾臂指数和血尿酸水平与糖尿病周围神经病变(DPN)的相关性。方法:选取2018年8月至2019年8月在社区医院治疗并建立档案的2型糖尿病(T2DM)患者500例,根据有无糖尿病周围神经变(DPN)分为有DPN组及无DPN组,分别为272例和228例。分别统计各组的血尿酸(UA)和趾臂指数(TBI)水平。探讨趾臂指数(TBI)及血尿酸与T2DM患者周围神经病变的发生发展的相关性。结果:DPN组TBI水平显著低于对照组,而UA显著高于非DPN组比较,差异具有统计学意义(P<0.05)。单因素分析显示:DPN组TBI、UA异常率(62.28%、46.49%)与非DPN组(33.09%、23.90%)比较,差异具有统计学意义(P<0.05),TBI和UA与DPN存在明显的相关性,且呈正相关性,DPN患者TBI、UA异常率明显升高。结论:随着TBI的降低及血尿酸水平的升高,T2DM患者周围神经病变的患病率逐渐增高,TBI下降及高尿酸血症是DPN发生发展的危险因素。
Objective:To explore the correlation between toe arm index,serum uric acid level and diabetic peripheral neuropathy(DPN)in community type 2 diabetic patients.Methods:from August 2018 to August 2019,500 patients with type 2 diabetes mellitus(T2DM)who were treated in the community hospital and established a file were divided into two groups according to whether there was DPN or not:group with DPN and group without DPN,272 cases and 228 cases respectively.The levels of UA and TBI were measured.To explore the relationship between toe arm index(TBI),serum uric acid and the development of peripheral neuropathy in T2DM patients.Results:TBI level of DPN group was significantly lower than that of control group,while UA was significantly higher than that of non DPN group(P<0.05).Univariate analysis showed that the abnormal rate of TBI and UA in DPN group(62.28%,46.49%)was significantly higher than that in non DPN group(33.09%,23.90%)(P<0.05),TBI and UA were positively correlated with DPN,and the abnormal rate of TBI and UA in DPN patients was significantly increased.Conclusion:with the decrease of TBI and the increase of serum uric acid level,the prevalence of peripheral neuropathy in T2DM patients increases gradually.The decrease of TBI and hyperuricemia are the risk factors for the occurrence and development of DPN.
