防旋股骨近端髓内钉内固定治疗老年人股骨转子间骨折的临床观察

Clinical Observation of Anti-Rotation Proximal Femoral Nail Internal Fixation for the Treatment of Intertrochanteric Fractures in the Elderly

目的:探讨PFNA(防旋股骨近端髓内钉内固定)治疗老年人股骨转子间骨折患者的临床价值。方法:研究时间为2014年10月至2018年10月,病例数共80例,均为股骨转子间骨折患者,随机分组各40例,对照组和观察组分别行动力髋螺钉(DHS)和PFNA治疗,评价两组疗效及并发症发生情况,记录手术创伤。结果:观察组手术时间、切口长度短于对照组,术中出血量低于对照组,P<0.05;观察组治疗优良率为90.00%,高于对照组的72.50%,P<0.05;观察组并发症发生率5.00%,低于对照组的20.00%,P<0.05。结论:针对股骨转子间骨折老年患者采用手术治疗具有良好效果,选择PFNA治疗方案可减少手术创伤及出血量,增强关节功能,而且并发症少,治疗效果好,安全性高,值得推广。

Objective:To explore the clinical value of PFNA(anti-rotation proximal femoral nail internal fixation)in the treatment of elderly patients with femoral intertrochanteric fractures.Methods:The study period was from October 2014 to October 2018.A total of 80 cases were patients with femoral intertrochanteric fractures.40 cases were randomly divided into groups.The control group and the observation group were driven hip screws(DHS)and PFNA respectively.Treatment,evaluate the efficacy and complications of the two groups,and record the surgical trauma.Results:The operation time and incision length of the observation group were shorter than those of the control group,and the amount of intraoperative blood loss was lower than that of the control group,P<0.05;the treatment excellent rate of the observation group was 90.00%higher than that of the control group 72.50%,P<0.05;the observation group had complications.The incidence rate of 5.00%was lower than 20.00%of the control group,P<0.05.Conclusion:Surgical treatment for elderly patients with femoral intertrochanteric fracture has good results.Choosing PFNA treatment can reduce surgical trauma and bleeding,enhance joint function,and has fewer complications,high therapeutic effect and safety,and it is worthy of promotion.
