纤支镜联合CYFRA21-1、CEA和CA125在肺癌诊断中的临床价值

Clinical value of bronchoscopy combined with CYFRA21-1,CEA and CA125 in the diagnosis of lung cancer

目的分析纤支镜联合细胞角蛋白19(CYFRA21-1)、癌胚抗原(CEA)和糖类抗原(CA125)在肺癌诊断的应用效果及临床价值。方法选取我院2018年1月至2020年1月收治的疑似肺癌的患者100例,对患者实施纤支镜与痰液肿瘤标志物检查,最后经病理学诊断为肺癌患者36例,良性肿瘤患者64例,对比两种检测方法的诊断准确性。结果纤支镜在肺癌诊中的阳性率为44.44%,与CEA、CYFRA21-1、CA125的阳性检出率相比不具有差异性(P>0.05)。相比于血液肿瘤标志物联合检测,纤支镜与肿瘤标志物联合检测的准确率更高(P<0.05)。肺癌与良性肿瘤患者的痰液肿瘤标志物相比有差异性(P<0.05)。结论纤支镜联合CYFRA21-1、CEA和CA125在肺癌疾病诊断中有着较高的诊断准确率,且可以对病理分期以及病理类型等进行有效鉴别,有助于临床上治疗该疾病。

Objective To analyze the application effect and clinical value of bronchoscopic combined cytokeratin 19(CYFRA21-1),carcinoembryonic antigen(CEA)and carbohydrate antigen(CA125)in the diagnosis of lung cancer.Methods 100 patients with suspected lung cancer admitted from January 2018 to January 2020 in our hospital were selected,and all patients were examined by bronchoscopy and sputum tumor markers.Finally,36 patients with lung cancer were diagnosed by pathology,benign tumor 64 patients were compared with the diagnostic accuracy of the two detection methods.Results After analysis,the positive rate of bronchoscopy in the diagnosis of lung cancer was 44.44%,which was not different from the positive detection rates of CEA,was CYFRA21-1,and CA125(P>0.05).After analysis,compared with the combined detection of blood tumor markers,the accuracy of combined detection of bronchoscopy and tumor markers was high(P<0.05).Lung cancer was different from benign tumor patients'sputum tumor markers(P<0.05).Conclusion The combination of bronchoscopy with CYFRA21-1,CEA and CA125 has a high diagnostic accuracy in the diagnosis of lung cancer,and can effectively identify the pathological stage and pathological type,which is helpful for clinical treatment of the disease.
