COPD合并OSAS患者发生呼吸衰竭的影响因素分析

Analysis on Influencing Factors of Occurrence of Respiratory Failure in Patients with COPD Complicated with OSAS

目的分析COPD合并OSAS患者发生呼吸衰竭的影响因素。方法回顾性分析2018年4月至2019年4月我院收治的82例COPD合并OSAS患者的临床资料,根据是否发生呼吸衰竭分为发生组(n=41)和未发生组(n=41)。经单因素、多因素分析探讨COPD合并OSAS患者发生呼吸衰竭的影响因素。结果发生组患者的性别、吸烟史、合并心脑血管疾病、咳嗽、发热、AHI、LAT及LSaO2与未发生组比较,差异不显著(P>0.05);发生组患者的年龄、颈围、呼吸困难、mSaO2与未发生组比较,差异显著(P<0.05);Logistic多因素回归分析结果显示,年龄、颈围、呼吸困难、mSaO2均为影响COPD合并OSAS患者发生呼吸衰竭的危险因素(OR>1,P<0.05)。结论COPD合并OSAS患者发生呼吸衰竭受多种因素影响,临床应针对危险因素积极采取有效预防措施,以提高治疗效果,改善患者预后。

Objective To analyze the influencing factors of occurrence of respiratory failure in patients with COPD complicated with OSAS.Methods The clinical data of 82 patients with COPD complicated with OSAS admitted to our hospital from April 2018 to April 2019 were retrospectively analyzed and were divided into occurrence group(n=41)and non-occurrence group(n=41)according to the occurrence of respiratory failure or not.The influencing factors of respiratory failure in patients with COPD complicated with OSAS were studied by single factor analysis and multi-factor analysis.Results Gender,smoking history,complication of cardiovascular and cerebrovascular diseases,cough,fever,AHI,LAT and LSaO2 in the occurrence group were not significantly different from those in the non-occurrence group(P>0.05).The age,neck circumference,dyspnea,and mSaO2 in the occurrence group were significantly different from those in the non-occurrence group(P<0.05).The results of Logistic multivariate regression analysis showed that age,neck circumference,dyspnea,and mSaO2 were risk factors influencing the occurrence of respiratory failure in patients with COPD complicated with OSAS(OR>1,P<0.05).Conclusions The occurrence of respiratory failure in patients with COPD complicated with OSAS is affected by many factors,and effective prevention measures should be taken according to the risk factors to improve the treatment effect and prognosis of patients.
