乳腺癌化疗病人症状群与生存质量的相关性研究

Correlation between symptom clusters and quality of life in patients with breast cancer undergoing chemotherapy

[目的]探讨乳腺癌化疗病人症状群的组成及对生存质量的影响。[方法]便利抽样法选取山东省肿瘤医院240例乳腺癌化疗病人作为研究对象,采用一般资料调查表、乳腺癌化疗病人症状测评量表、生存质量量表(FACT-G)对乳腺癌化疗病人进行调查。[结果]因子分析显示,乳腺癌化疗病人存在神经心理症状群、消化道症状群、自我形象紊乱症状群、激素相关症状群4个相关症状群,累计方差贡献率为69.13%。乳腺癌化疗病人症状群与生存质量均呈负相关(r值分别为-0.660,-0.484,-0.382,-0.152,P<0.05)。[结论]乳腺癌化疗病人症状发生率高,存在4个症状群,影响病人的生存质量,医护人员应关注病人症状群的发生发展。从症状群的管理角度出发,制定合理的干预措施,改善病人的生存质量。

Objective:To explore the composition of the symptom clusters of breast cancer patients undergoing chemotherapy and its impact on the quality of life.Methods:By convenience sampling,a total of 240 patients with breast cancer undergoing chemotherapy were selected from Shandong Cancer Hospital.The general information questionnaire,symptom assessment scale of breast cancer patient undergoing chemotherapy,and Functional Assessment of Cancer Therapy-General(FACT-G)scale were used to investigate patients.Results:Factor analysis showed that there were neuropsychological symptom clusters,digestive tract symptom clusters,self-image disorder symptom clusters,hormone-related symptom clusters in patients with breast cancer undergoing chemotherapy.The cumulative variance contribution rate was 69.13%.The symptom clusters were negatively correlated with the quality of life in patients with breast cancer undergoing chemotherapy(r=-0.66,-0.49,-0.38,-0.15,P<0.05).Conclusions:The incidence of symptoms in breast cancer patients undergoing chemotherapy was high.There are 4 symptom clusters,which affect the quality of life of patients.Medical staff should pay attention to the occurrence and development of symptom clusters.From the perspective of symptom group management,the reasonable intervention measures were formulated to improve the quality of life of patients.
