输入型或家庭聚集发病的新型冠状病毒性肺炎CT诊断

摘要 目的:通过对2019新型冠状病毒性肺炎(COVID-19)患者的胸部CT影像表现回顾,提炼COVID-19的影像学特征,帮助临床快速识别和早期诊断。方法:回顾性分析本院收治的5例输入型或家庭聚集发病的COVID-19患者的临床和胸部影像资料,分别对性别、年龄、接触史、症状及胸部CT的病灶表现、累及肺叶数量和分布、其他表现进行分析。结果:已确诊的5例COVID-19患者在初次胸部CT检查时均有异常,3例右肺上叶病变,2例右肺中叶病变,4例右肺下叶病变,4例左肺上叶病变,3例左肺下叶病变。其中2例累及单侧肺叶,3例累及双侧肺叶。4例病灶主要位于胸膜下;从病灶密度看,4例呈有磨玻璃密度改变,1例实变,3例两种病变均有;从病灶形态看,病灶主要呈磨玻璃影和实变影。其中1例患者的胸部CT随访显示疾病的轻度或中度进展,表现为肺混浊程度和密度的增加。结论:新型冠状病毒性肺炎的胸部CT表现具有一定的特征性,识别这些特征,放射科医师可对该病做出快速识别和早期诊断,帮助临床诊疗。 Objective:By reviewing the chest CT findings of patients with novel coronavirus pneumonia 2019(COVID-19),extract the imaging features of COVID-19,help clinical rapid recognition and early diagnosis.Methods:The clinical and chest imaging data of 5 patients with COVID-19 were analyzed retrospectively,including sex,age,contact history,symptoms,CT findings,number and distribution of involved lung lobes,and other manifestations.Results:Five confirmed patients with COVID-19 had abnormalities in the first chest CT examination,including 3 lesions in the right upper lobe,2 lesions in the right middle lobe,4 lesions in the right lower lobe,4 lesions in the left upper lobe,and 3 lesions in the left lower lobe.Among them,2 cases involved unilateral lobes and 3 cases involved bilateral lobes.The lesions were mainly located under pleura in 4 cases,ground glass density in 4 cases,consolidation in 1 case and both in 3 cases.Ground glass shadow and consolidation shadow were mainly seen in the lesions.CT follow-up of one patient showed mild or moderate progress of the disease,which showed the increase of lung turbidity and density.Conclusion:The chest CT findings novel coronavirus pneumonia have certain characteristics.To identify these characteristics,radiologists can quickly identify and early diagnose the disease,and help clinical diagnosis and treatment.
