分析多根多发性肋骨骨折手术中临床应用全身麻醉联合椎旁神经阻滞麻醉镇痛的效果

To Analyze the Effect of Clinical General Anesthesia Combined with Paravertebral Nerve Block Anesthesia in the Operation of Multiple Multiple Rib Fractures

目的分析多根多发性肋骨骨折手术中临床应用全身麻醉联合椎旁神经阻滞麻醉镇痛的效果。方法选取贵州医科大学第三附属医院2018年7月至2019年12月收治并接受多发性肋骨骨折手术的患者76例为研究对象,按不同的麻醉镇痛方式分为对照组(38例)与干预组(38例),对照组进行全身麻醉,干预组进行全身麻醉联合椎旁神经阻滞麻醉镇痛,麻醉后对结果分析和总结。结果干预组的不良反应率低于对照组,差异有统计学意义(χ2=6.98,P<0.05)。干预组的各项评分值低于对照组。干预组的术后清醒时间短于对照组,枸椽酸舒芬太尼少于对照组,差异均有统计学意义(P<0.05)。结论针对多发性肋骨骨折手术的患者实施全身麻醉联合椎旁神经阻滞麻醉,整体上有麻醉和超前镇痛重要的作用,可以减少全麻药用量和不良反应,提升了整体康复效果。

Objective To analyze the analgesic effect of general anesthesia combined with paraspinal nerve block in the operation of multiple rib fractures.Methods A total of 76 cases of multiple rib fractures treated in the third Affiliated Hospital of Guizhou Medical University from July 2018 to December 2019 were selected as subjects,and divided into control group and intervention group with different anesthesia and analgesia methods.The control group received general anesthesia,and the intervention group received general anesthesia combined with paravertebral nerve block anesthesia and analgesia.The results were analyzed and summarized after anesthesia.Results By comparing and analyzing the adverse reaction rate of the two groups,the adverse reaction rate of the intervention group was lower than that of the control group,5.2%and 15.8%,respectively,χ2=6.98,the difference was statistically significant(P<0.05).The VAS scores of the two groups of cases in different time periods were compared and analyzed.The scores of the intervention group were lower than those of the control group.Postoperative waking time was shorter in the intervention group than in the control group,and sufentanil citrate was less than in the control group.The difference was statistically significant(P<0.05).Conclusion The general anesthesia combined with paravertebral nerve block anesthesia for cases of multiple rib fracture surgery plays an important role in anesthesia and preemptive analgesia on the whole,which can reduce the dosage of general anesthetics and adverse reactions,and improve the overall rehabilitation effect.
