数字化3D打印技术治疗复杂髋臼骨折的效果及对肢体功能的影响 被引量:6

摘要 目的:探究利用数字化三维(3D)打印技术对复杂髋臼骨折患者进行手术治疗的效果,以及对患者术后肢体功能恢复的影响。方法:选取在医院就诊的100例行手术治疗的髋臼骨折患者,按照随机数表法将其分为对照组和观察组,每组50例。对照组采用传统手术方法进行治疗,观察组借助数字化3D打印技术制造髋臼骨折模型确定手术入路方式进行手术治疗。术后2 d,评判两组患者复位效果,观察比较两组患者的术中出血量、手术时间、切口长度、术后引流量和并发症发生率;随访术后1年两组患者髋关节功能恢复情况。结果:术后2 d观察组患者治疗有效率为92%,显著优于对照组的74%,且并发症发生率低于对照组,其差异有统计学意义(x^2=5.741,x^2=7.484;P<0.05);观察组患者的术中出血量、手术时间、切口长度和术后引流量指标均显著低于对照组,其差异有统计学意义(t=4.290,t=6.569,t=12.921,t=18.646;P<0.05);两组患者术后1年髋关节功能的恢复情况比较,观察组患者优良率为90.0%,显著优于对照组(68.0%),其差异有统计学意义(x^2=7.294,P<0.05)。结论:数字化3D打印技术应用于复杂髋臼骨折患者的手术,可以降低手术难度,缩短手术时间,降低并发症的发生率,提高患者髋关节功能的恢复速度,具有良好的临床应用价值。 Objective:To explore the effect of digital three-dimensional(3D)printing technique on the surgical treatment of patients with complex fracture of acetabulum and its influence on the recovery of limb function post operation of patients.Methods:100 patients with fracture of acetabulum who received surgical treatment in the hospital were randomly divided into control group and observation group,with 50 cases in each group.The control group was treated with traditional surgical methods,and the observation group was treated with digital 3D printing technique that could produce the model of the fracture of acetabulum to determine the surgical approach for surgical treatment.At the 2th day after the operation,the reduction effect of the two groups was assessed,and the intraoperative blood loss,operation time,incision length,postoperative drainage volume and incidence of complications between the two groups were observed and compared.And the recovery of the function of hip joint of the two groups was followed up for 1 year.Results:At the 2th day after the operation,the effective rate of the observation group was 92%,which was significantly better than 74%of the control group,and the incidence of complications of observation group was significantly lower than that of control group(x^2=5.741,x^2=7.484,P<0.05).And the intraoperative blood loss,operation time,incision length and postoperative drainage index of observation group were significantly lower than those of control group(t=4.290,t=6.569,t=12.921,t=18.646,P<0.05),respectively.The recovery of the function of hip joint at the 1st year post operation of observation group(90.0%)was significantly better than that of control group(68.0%)(x^2=7.294,P<0.05).Conclusion:The application of digital 3D printing technique in the operation of patients with complex fracture of acetabulum can reduce the difficulty of operation,and shorten the operation time,and reduce the incidence of complications,and improve the recovery speed of the function of hip joint,which has favorable clinical application value.
