高频彩色多普勒超声对护理人员足踝部稳定性研究

Study on the stability of foot and ankle of paramedics by high frequency color Doppler ultrasound

目的:探究高频彩色多普勒超声对护理人员足踝部稳定性诊断的价值。方法:选取60例疑似足踝部创伤性病变的临床护理人员,均采用磁共振成像(MRI)、X射线及高频彩色多普勒超声进行检查,观察超声声像图的改变特征,将超声检查结果分别与MRI、X射线检查及手术病理结果进行比较。结果:超声诊断与MRI诊断肌腱损伤的结果相比,诊断的符合率为95.00%;与X射线诊断足踝部骨折的结果相比,诊断的符合率为93.33%,两种方法具有较好的诊断一致性(Kappa=0.862)。Ⅰ型、Ⅱ型及Ⅲ型韧带损伤的符合率依次为66.67%、76.47%和83.33%,总体符合率为77.42%%,且与手术病理结果基本吻合;以手术病理结果和MRI诊断结果为金标准,高频彩色多普勒超声诊断的灵敏度为88.26%,特异度为77.33%,阳性预测值为85.72%,阴性预测值为81.05%。结论:高频彩色多普勒超声对护理人员足踝部稳定性具有较高的诊断价值,为临床诊断和治疗足踝部创伤性病变提供了更充足的依据。

Objective:To explore the value of high frequency color Doppler ultrasound in diagnosing the stability of foot and ankle of paramedics.Methods:60 clinical paramedics with suspected trauma on foot and ankle were selected and they were examined by magnetic resonance imaging(MRI),X-ray and high-frequency color Doppler ultrasound.The change characteristics of ultrasonic images were observed,and the results of ultrasound examination were compared with MRI,X-ray examination and the results of surgical pathology.Results:The coincidence rate of ultrasound diagnosis and MRI diagnosis of tendon injury was 95.00%,and that of ultrasound diagnosis and X-ray diagnosis of foot and ankle fracture was 93.33%.The two methods had good diagnostic consistency(kappa=0.862).The coincidence rates of type I,type II and type III ligament injury were 66.67%,76.47%and 83.33%respectively,and the overall coincidence rate was 77.42%,which was basically consistent with the results of surgical pathology.The surgical pathological results and MRI diagnosis results were used as the gold standard,and the sensitivity,specificity,positive predictive value and negative predictive value of high-frequency color Doppler ultrasound diagnosis were 88.26%,77.33%,85.72%and 81.05%,respectively.Conclusion:High frequency color Doppler ultrasound has higher diagnostic value for the stability of the foot and ankle of paramedics,and can provide more sufficient basis for clinical diagnosis and treatment on traumatic lesions of foot and ankle.
