口腔癌患者术后中西医结合治疗效果观察

The effects of combined traditional Chinese and western medicine on postoperative oral cancer patients

目的:探讨口腔癌患者术后中西医结合治疗效果。方法:纳入62例口腔癌患者,分为观察组和对照组(n=31),另选30例健康志愿者作为健康组。对照组术后予以顺铂、5-氟尿嘧啶持续动脉灌注化疗,观察组于对照组基础上予以通络解毒法治疗。对比2组临床疗效、毒副反应发生率、生存质量改善率、治疗前与疗程结束后血清血管内皮生长因子(VEGF)、碱性成纤维细胞生长因子(bFGF)、白细胞介素(IL)-6、IL-8、免疫功能指标(CD4^+、CD8^+、CD56^+、CD4^+/CD8^+)水平。结果:治疗后,观察组与对照组血清bFGF、VEGF、IL-6、IL-8水平较治疗前下降(P<0.05),观察组与健康组比较(P>0.05),对照组高于健康组(P<0.05),观察低于对照组(P<0.05);2组均无Ⅳ度毒副反应,且观察组不良反应发生率低于对照组(P<0.05);观察组与对照组CD8^+、CD56^+较治疗前下降(P<0.05),CD4^+、CD4^+/CD8^+较治疗前升高(P<0.05),观察组与健康组相比(P>0.05),对照组CD8^+、CD56^+仍高于健康组(P<0.05),CD4^+、CD4^+/CD8^+低于健康组(P<0.05),观察组血清CD4^+、CD4^+/CD8^+水平高于对照组(P<0.05),CD8^+、CD56^+水平低于对照组(P<0.05);3个疗程后,观察组KPS高于对照组(P<0.05)。结论:中西医结合治疗应用于口腔癌患者术后,可有效调节患者免疫功能,降低血清VEGF、bFGF水平。

Objective:To investigate the effects of combined traditional Chinese and western medicine on postoperative patients with oral cancer.Methods:62 patients with oral cancer were randomly divided into observation and control groups(n=31).30 healthy volunteers were included into the healthy control group.The patients in control group received continuous arterial infusion chemotherapy with cisplatin and 5-fluorouracil after operation,those in observation group were treated with Tongluo detoxification based on the treatment of control group.The clinical efficacy,incidence of adverse reaction,improvement in quality of life(KPS),serum VEGF,bFGF,IL-6,IL-8 and the level of CD4^+,CD8^+,CD56^+,CD4^+/CD8^+before and after treatment were statistically compared between the groups.Results:After treatment the serum bFGF,VEGF,IL-6,and IL-8 levels in the 2 groups of patients were lower than those before treatment(P<0.05),between the observation group and the healthy group,P>0.05,those of control group was still higher than those of the healthy group(P<0.05),and those of the observation was lower than those of the control group(P<0.05).NoⅣdegree adverse reaction was not observed in both groups,and the incidence of adverse reaction was lower in the observation group than that in the control group(P<0.05).At the end of the treatment period,the CD8^+and CD56^+of the observation group and the control group were decreased(P<0.05),CD4^+,CD4^+/CD8^+were increased(P<0.05),between the observation group and the healthy group,P>0.05,CD8^+and CD56^+in the control group were still higher than those in the healthy group(P<0.05),CD4^+,CD4^+/CD8^+were lower than those in the healthy group(P<0.05),the levels of serum CD4^+,CD4^+/CD8^+in the observation group were higher than those in the control group,and the levels of CD8^+and CD56^+in the observation group were lower than those in the control group(P<0.05).After three courses of treatment,KPS of the observation group was higher than that of the control group(P<0.05).Conclusion:Integrated Chinese and western medicine can effectively regulate immune function,reduce serum VEGF and bFGF levels of the postoperative patients with oral cancer.
