火针疗法治疗风寒湿痹型冻结肩临床研究

Clinical Study on Fire Acupuncture in Treating Frozen Shoulder of Bi Syndrome of Wind,Cold and Dampness

目的:观察火针疗法治疗风寒湿痹型冻结肩(急性期)的临床疗效。方法:收集2018年9月—2019年8月我院针灸科就诊冻结肩患者60例,将患者按随机数字表法随机分为治疗组和对照组。对照组给予普通针刺,治疗组给予火针疗法。比较两组患者血清降钙素基因相关肽(CGRP)水平及患者治疗后的疗效、视觉模拟(VAS)评分、肩关节周围炎评定量表的变化。结果:治疗组和对照组治疗后,血浆CGRP水平、VAS评分均较治疗前下降,肩关节周围炎评定量表各项评分均较治疗前上升,且治疗组治疗后血浆CGRP水平及VAS评分低于对照组,日常生活能力评分、肩关节活动功能评分、肩关节疼痛评分及肩关节周围炎评定量表总分高于对照组(P<0.05)。治疗组总有效率93.33%(28/30),高于对照组的73.33%(22/30),差异具有统计学意义(P<0.05)。结论:火针疗法用于风寒湿痹型冻结肩急性期可降低血浆CGRP的表达水平,减轻患者疼痛,改善肩关节活动功能,提高日常生活能力。

Objective:To observe the clinical effect of fire acupuncture on frozen shoulder(acute phase)of Bi syndrome of wind,cold and dampness.Methods:60 patients with frozen shoulder were randomly divided into the treatment group and the control group.The control group was treated with ordinary acupuncture,and the treatment group was treated with fire acupuncture.The serum level of calcitonin gene-related peptide(CGRP),the efficacy,the scores of VAS and scapulohumeral periarthritis scale were compared between the two groups after the treatment.Results:The level of CGRP and the scores of VAS were decreased,and the scores of the scapulohumeral periarthritis scale were increased in the two groups after the treatment;of which the improvements in the treatment group were more significant(P<0.05).The total effective rate was 93.33%(28/30)in the treatment group,which was higher than 73.33%(22/30)in the control group(P<0.05).Conclusion:The fire acupuncture therapy can reduce the expression level of plasma CGRP in the acute stage of frozen shoulder of Bi syndrome of wind,cold and dampness,it can reduce the pain,improve the function of the shoulder joint and improve the daily living ability of the patients.
