基于数据挖掘的针灸治疗面肌痉挛选穴规律分析

Law of Acupoint Selection in the Treatment of Hemifacial Spasm Based on Data Mining Method

目的:运用古今医案云平台(V1.6.3)数据挖掘技术,探讨面肌痉挛针刺治疗的选穴规律及核心处方。方法:检索包括中国知网(CNKI)、万方数据库(WF)、维普(VIP)建库至2019年5月30日发表的针灸治疗面肌痉挛相关文献。采用平台对纳入处方穴位频次、穴对分析、关联规则分析、复杂网络分析等模块进行分析。结果:纳入文献84篇,合计87首处方,涉及腧穴81个。其中取穴频次前3位的腧穴为合谷(67.82%)、地仓(57.47%)、四白(54.02%);前3位的穴对有“合谷-太冲”“四白-地仓”“颊车-地仓”;核心穴位组合为合谷、太冲、风池、地仓、下关、颧髎、颊车、太阳和四白。结论:针刺治疗面肌痉挛选穴遵循“局部与远端取穴配合,祛邪与扶正并重”,达到调和气血、熄风通络止痉的作用,为该病临床治疗提供有效依据。

Objective:To explore the law of acupoints selection and acupoints prescription in the treatment of hemifacial spasm with the ancient and modern medical case cloud platform(V1.6.3).Methods:Literature about TCM treatment for hemifacial spasm in CNKI,VIP and Wanfang databases were retrieved from establishment to 30th May,2019.The platform was used to analyze the frequency of points,couple points analysis,association rule analysis and complex network analysis.Results:A total of 84 articles were included,covering 87 prescriptions and involving 81 acupoints.Among them,the top three acupoints were Hegu(LI4)(67.82%),Dicang(ST4)(57.47%)and Sibai(ST3)(54.02%);the first three couple points were LI4-Taichong(Liv3),ST3-ST4 and Jiache(ST6)-ST4;core points were LI4,Liv3,Fengchi(GB20),ST4,Xiaguan(ST7),Quanliao(SI18),ST6,Taiyang(EX-HN5)and ST3.Conclusion:The rule of acupoint selection in the treatment of hemifacial spasm is the combination of local and distal acupoints,eliminating pathogenic factors and strengthening vital-qi equally,so as to achieve the effect of harmonizing qi and blood,calming wind,dredging collaterals and stopping spasm,which provides effective evidence for the clinical treatment of the disease.
