丁苯酞联合尿激酶静脉溶栓治疗对急性缺血性脑卒中患者神经功能缺损程度的影响

Effects of butylphthalide combined with urokinase intravenous thrombolysis on neurologic impairment degree in patients with acute ischemic cerebral stroke

目的探讨丁苯酞联合尿激酶静脉溶栓治疗对急性缺血性脑卒中患者临床神经功能缺损程度的影响。方法选取2017年10月至2018年12月因急性缺血性脑卒中入院治疗的168例患者,将其分为对照组与试验组84例。对照组采用尿激酶静脉溶栓+常规治疗,试验组采用对照组基础上+丁苯酞治疗,比较2组患者的神经功能缺损程度。结果试验组临床有效率(92.86%)明显优于对照组的有效率(78.57%),差异有统计学意义(χ^2=7.00,P<0.01);2组患者治疗前NIHSS评分,差异无统计学意义(t=0.125,P=0.948);治疗后试验组NIHSS评分明显低于对照组,差异有统计学意义(t=4.884,P=0.000);治疗前2组患者ADL评分,差异无统计学意义(t=0.021,P=0.417);研究组ADL评分明显优于对照组ADL评分(t=5.454,P=0.000),差异有统计学意义(P<0.05)。结论丁苯酞联合尿激酶静脉溶栓治疗缺血性脑卒中,可有效的提高临床疗效,改善神经功能缺损状况,值得临床推广应用。

Objective To investigate the effects of butylphthalide combined with urokinase intravenous thrombolysis on neurologic impairment degree in patients with acute ischemic cerebral stroke.Methods A total of 168 patients with acute ischemic cerebral stroke who were admitted and treated in our hospital from October 2017 to December 2018 were enrolled in the study,who were randomly divided into control group and experimental group,with 84 patients in each group.The patients in control group were treated by urokinase intravenous thrombolysis+routine treatment,however,the patients in experimental group,on the basis of control group,were treated by butylphthalide.The changes of neurologic impairment degree were observed and compared between the two groups.Results The effective rate in experimental group was 92.86%,which was significantly higher than that(78.57%)in control group(P<0.01).Before treatment there were no significant differences in the NIHSS scores between the two groups(P>0.05).After treatment the NIHSS scores in experimental group were significantly lower than those in control group(P<0.01).Before treatment there were no significant differences in ADL NIHSS scores between the two groups(P>0.05).After treatment the ADL scores in experimental group were significantly better than those in control group(P<0.01).Conclusion The butylphthalide combined with urokinase intravenous thrombolysis in treatment of ischemic cerebral stroke can effectively improve the clinical efficacy and improve the status of neurologic impairment,therefor,which is worthy of clinical application and promotion.
