摘要
目的探讨多层螺旋CT三维重建技术在外科急腹症临床诊断与治疗中的应用价值。方法选取2018年1月—2019年1月急诊收治的疑似外科急腹症患者186例作为研究对象。所有研究对象均接受腹部螺旋CT检查,根据CT原始影像资料及三维重建后处理图像进行诊断,并与临床确诊结果进行比较。结果186例疑似外科急腹症患者经临床证实确诊为外科急腹症147例,其中急性胰腺炎25例,肝胆系急腹症18例,急性阑尾炎32例,消化道穿孔21例,肠梗阻29例,泌尿系急腹症22例。CT对186例疑似外科急腹症患者诊断准确率为83.33%,灵敏度为85.03%,特异度为76.92%。结论多层螺旋CT三维重建技术对各种类型的外科急腹症诊断均具有较高的准确率与灵敏度,有助于外科急腹症的早期诊断与鉴别诊断,对临床治疗方案的快速、准确制定具有重要的临床价值。
Objective To explore the application value of multi-slice spiral CT 3D reconstruction in clinical diagnosis and treatment of surgical acute abdomen.Methods A total of 186 patients with suspected surgical acute abdomen admitted to emergency department from January 2018 to January 2019 were selected.All subjects underwent abdominal spiral CT examination,and were diagnosed according to the original CT images and three-dimensional reconstruction images,and compared with the clinical results.Results 186 cases of suspected surgical acute abdomen were clinically confirmed as surgical acute abdomen in 147 cases,including 25 cases of acute pancreatitis,18 cases of hepatobiliary acute abdomen,32 cases of acute appendicitis,21 cases of digestive tract perforation,29 cases of intestinal obstruction,acute abdomen of urinary system:A report of 22 cases.The diagnostic accuracy,sensitivity and specificity of CT were 83.33%,85.03%and 76.92%in 186 patients with suspected surgical acute abdomen.Conclusion Multi-slice spiral CT 3D reconstruction has high accuracy and sensitivity in the diagnosis of various types of surgical acute abdomen,which is helpful to the early diagnosis and differential diagnosis of surgical acute abdomen,it has important clinical value for the rapid and accurate establishment of clinical treatment plan.
