精细化护理对冠心病心力衰竭患者心功能及运动耐力的影响

The influences on heart function and exercise endurance of elaborate nursing for patients with coronary heart disease and heart failure

目的:应用精细化护理对冠心病心力衰竭患者进行护理干预,评估其对患者心功能、运动耐力及护理质量影响。方法:按数字随机法,将90例冠心病心力衰竭患者分为对照组和研究组,每组各45例。对照组应用常规护理,研究组应用精细化护理进行干预。评估患者左室射血分数(LVEF)、每搏心输出量(SV),检测血浆B型脑钠肽(BNP)水平,评估患者6 min步行距离,应用明尼苏达心力衰竭生活质量调查表(MLWHFQ)评估患者生活质量,并评估护理质量评分、患者依从性评分、患者满意度评分。结果:干预后,研究组LVEF(%)、SV、6 min步行距离、护理质量评分、患者依从性评分、患者满意度评分分别为(53.08±0.69)%、(42.59±0.75)mL/次、(302.27±12.95)m、(94.73±5.89)分、(88.60±3.72)分、(96.53±4.51)分,均高于对照组的(47.52±0.76)%、(38.76±0.68)mL/次、(275.81±13.94)m、(86.02±6.31)分、(71.45±3.96)分、(82.19±4.70)分,差异有统计学意义(P<0.05);干预后,研究组NYHA分级、血浆BNP、MLWHFQ评分分别为(2.20±0.19)、(283.52±23.90)pg/mL、(45.29±3.49)分,均低于对照组的(2.73±0.20)、(379.49±24.79)pg/mL、(53.48±3.50)分,差异均有统计学意义(P<0.05)。结论:应用精细化护理对冠心病心力衰竭患者进行护理干预,能够改善患者心功能,提高患者运动耐力及护理质量影响。

Objective:Application of elaborate nursing care for coronary heart disease patients with heart failure,and to evaluate the impact on cardiac function,to exercise endurance and nursing quality.Methods:90 coronary heart disease patients with heart failure were randomly divided into control group and study group,45 cases in each group.The control group used routine nursing and study group used elaborate nursing intervention.Assessed the patient s left ventricular ejection fraction(LVEF),stroke volume(SV),detected plasma BNP levels,assessed the patient s 6-minute walking distance,and assessed quality of life use the Minnesota Living with Heart Failure Questionnaire(MLWHFQ),and evaluated the nursing quality score,patient compliance score,and patient satisfaction score.Results:After intervention,the LVEF(%),SV,6min walking distance,nursing quality score,patient compliance score and patient satisfaction score of the study group were(53.08±0.69)%,(42.59±0.75)mL/time,(302.27±12.95)m,(94.73±5.89)points,(88.60±3.72)points,(96.53±4.51)points,which were higher than those in the control group(47.52±0.76)%,(38.76±0.68)mL/time,(275.81±13.94)m,(86.02±6.31)points,(71.45±3.96)points,(82.19±4.70)points,the differences were statistically significant(P<0.05).After intervention,the NYHA grade,plasma BNP,and MLWHFQ scores of the study group after treatment were(2.20±0.19),(283.52±23.90)pg/mL,(45.29±3.49)points,which were lower than those in the control group(2.73±0.20),(379.49±24.79)pg/mL,(53.48±3.50)points,the differences were statistically significant(P<0.05).Conclusion:The application of elaborate nursing intervention in patients with coronary heart disease and heart failure can improve the heart function of patients,improve the exercise endurance of patients and the influence of nursing quality.
