卡前列素氨丁三醇、卡孕栓和缩宫素联合治疗产后出血的效果

Effect of Carboprost Tromethamine,Carboprost Methylate Suppository and Oxytocin in the treatment of postpartum hemorrhage

目的研究卡前列素氨丁三醇、卡孕栓联合缩宫素治疗产后出血的临床效果。方法选取2017年5月~2019年4月我院收治的56例产后出血患者作为研究对象,按照随机数字表法分为观察组(28例)与对照组(28例)。对照组采取常规缩宫素联合卡孕栓治疗,观察组在前者基础上加用卡前列素氨丁三醇治疗。记录两组治疗前的出血量及产后2、24 h出血量,产后24 h血红蛋白下降值,比较两组的治疗有效率、不良反应总发生率以及治疗前后的心率、舒张压、收缩压水平变化情况。结果观察组的总有效率高于对照组,差异有统计学意义(P<0.05);观察组产后2、24 h的出血量及产后24 h血红蛋白下降量少于对照组,差异有统计学意义(P<0.05);治疗后,观察组的心率、收缩压、舒张压均高于对照组,差异有统计学意义(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论在产后出血患者的治疗中,采用卡前列素氨丁三醇、卡孕栓联合缩宫素治疗可显著减少产妇的产后出血量,有助于稳定患者的心率和血压等生命体征,疗效显著,安全性好。

Objective To study the clinical effects of Carboprost Tromethamine,Carboprost Methylate Suppository combined with Oxytocin in the treatment of postpartum hemorrhage.Methods A total of 56 patients suffering from postpartum hemorrhage admitted to our hospital from May 2017 to April 2019 were selected as the research subjects and divided into the observation group(n=28)and the control group(n=28)according to the random number table method.In the control group,conventional Oxytocin combined with Carboprost Methylate Suppository was used,and in the observation group,Carboprost Tromethamine was added on the basis of the control group.The bleeding volume before treatment,the bleeding volume at 2,24 h after delivery,and the decrease in hemoglobin at 24 h were recorded.The overall response rate,the total incidence of adverse reactions and the changes of heart rate(HR),diastolic blood pressure(DBP),and systolic blood pressure(SBP)before and after treatment in the two groups were compared.Results The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).The bleeding volume of the observation group at 2 h and 24 h after delivery and the decrease of hemoglobin at 24 h after delivery were less than those of the control group,and the differences were statistically significant(P<0.05).After treatment,the HR,SBP,and DBP of the observation group were higher than those of the control group,with statistical significance(P<0.05).There was no statistical difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion In the treatment of patients with postpartum hemorrhage,Carboprost Tromethamine,Carboprost Methylate Suppository combined with Oxytocin can remarkably reduce the amount of postpartum hemorrhage in puerpera and stabilize the patient′s vital signs such as HR and blood pressure.The curative effect is remarkable,the safety is good.
