输尿管导管联合输尿管镜在输尿管狭窄中的临床应用

Clinical application of ureteral catheter combined with ureteroscope in the treatment of ureterostenosis

目的探究在输尿管狭窄患者治疗中采用输尿管导管联合输尿管镜治疗的临床效果。方法研究对象为80例河源市源城区人民医院收治的输尿管狭窄患者,研究时间为2018年8月~2019年8月。将所有患者随机分为两组,对照组(40例)采用开放性手术治疗,观察组(40例)采用输尿管导管联合输尿管镜治疗。本研究随访3个月,将所有研究对象所反馈的数据进行比较分析,即治疗效果和手术并发症发生率。结果经过治疗后,观察组的治疗效果十分显著,尿流速快,即观察组患者手术后Qmax为(23.45±1.85)m L/s,对照组患者手术后Qmax为(18.98±2.21)m L/s,差异有统计学意义(P <0.05);经过治疗后,观察组患者的手术并发症发生较少,即术后出血1例(2.50%),对照组患者发生再次狭窄3例(7.50%)、术后感染3例(7.50%)、术后出血3例(7.50%),共计9例(22.50%),差异有统计学意义(P <0.05)。结论针对输尿管狭窄患者采用输尿管导管联合输尿管镜治疗,治疗效果明显,较传统治疗方法具有明显的优势,降低手术并发症发生率,被患者所认可,值得在临床上推广。

Objective To probe into clinical efficacy of ureteral catheter combined with ureteroscope in the treatment of the patients with ureterostenosis.Methods A total of 80 patients with ureterostenosis admitted to and treated in Yuancheng People’s Hospital of Heyuan City were selected as the subjects of research.The experimental period was from August 2018 to August 2019.All the patients were randomly divided into two groups,with the control group(n=40)treated with open operation and the observation group(n=40)treated with ureteral catheter combined with ureteroscope.The study was followed up for 3 m,and the data on the therapeutic effect and the incidence of operative complications fed back by all the subjects were compared and analyzed.Results After treatment,the therapeutic effect on the observation group was much significant:the urine flow rate was faster at Qmax was(23.45±1.85)m L/s after operation in the patients of the observation group than the Qmax was(18.98±2.21)m L/s after operation in the patients of the control group,and the difference was statistically significant(P<0.05).After treatment,the patients of the observation group had fewer operative complications with 1 case(2.50%)of postoperative hemorrhage,while the patients of the control group had 3 cases(7.50%)of restenosis,3 cases(7.50%)of postoperative infection,3 cases(7.50%)of postoperative hemorrhage,totaling 9 cases(22.50%)of operative complications,and the difference between the two groups was statistically significant(P<0.05).Conclusion Ureteral catheter combined with ureteroscope produces significant therapeutic effect on the patients with ureterostenosis.Compared with traditional therapies,it is obviously advantageous in that it can reduce the incidence of operative complications.Having won the patients’recognition,it is worthy of clinical promotion.
