超声及临床特征在附睾结核与非特异性附睾炎中的应用价值

The value of ultrasonography and clinical features in epididymal tuberculosis and nonspecific epididymitis

目的评价超声及临床特征在附睾结核与非特异性附睾炎中的应用价值。方法回顾性分析附睾结核、非特异性附睾炎各37例,分别从声像图表现、临床资料总结其特点。结果附睾结核病程较长,多以阴囊无痛性肿大来诊,声像图多表现为病变呈弥漫性分布,呈不均质低回声,且常出现钙化,血流不丰富;而非特异性附睾炎病程较短,多以阴囊肿痛来诊,声像图多表现为病变呈局限性分布,多位于附睾尾部,呈不均质低回声,血流丰富,且常合并阴囊壁增厚及精索增粗。两者相比差异具有统计学意义。结论超声表现及临床特征在附睾结核与非特异性附睾炎的鉴别诊断中具有重要价值,可为临床诊断提供客观依据。

Objective To evaluate the value of ultrasonography and clinical features in epididymal tuberculosis and nonspecific epididymitis.Methods Thirth-seven cases of epididymal tuberculosis and 37 cases of nonspecific epididymitis were analyzed retrospectively.The characteristics of epididymal tuberculosis and nonspecific epididymitis were summarized from their sonographic manifestations and clinical data.Results Epididymal tuberculosis had a long course of disease,which was usually diagnosed by painless swelling of the scrotum.The sonogram showed that the lesions were diffusely distributed,heterogeneous and hypoechoic,often calcified,and the blood flow was not abundant.The course of nonspecific epididymitis was short,which was usually diagnosed by swelling and pain of the scrotum.The sonogram showed that the lesions were localized,mostly in the tail of the epididymis,heterogeneous and hypoechoic,with rich blood flow.The scrotal wall and spermatic cord were often thickened.There was statistical significance of them.Conclusion Ultrasonography and clinical features are of great value in the differential diagnosis of epididymal tuberculosis and nonspecific epididymitis,which can provide objective basis for clinical diagnosis.
